轻型地中海贫血者的血液学实验参数研究

摘    要:目的探讨轻型地中海贫血的血液学检测参数变化及临床诊断意义。方法应用全自动血细胞分析仪检测红细胞各参数,用醋酸纤维薄膜电泳检测血红蛋白有无异常区带,一管法测定红细胞脆性,层析柱法测HbA2,抗碱法测HbF,Wright氏染色后镜检红细胞形态。结果对77例轻型地中海贫血、32例缺铁性贫血与27例健康人的检测结果显示,轻型地中海贫血者红细胞MCV及MCH等参数、红细胞脆性试验、HbA2及HbF与对照组有显著性意义(P<0.01)。结论上述相关血液学检测指标是筛查轻型地中海贫血的可靠指标,对Hb正常,RBC数高的疑似病人,应建议做地中海贫血基因检测,以进一步诊断。

Abstract:Objective To evaluate peripheral blood parameters for diagnostic significance of minor thalassemia. Methods Peripheral blood samples were detected from 77 cases with minor thalassemia, 32 cases with iron-deficiency anemia and 27 healthy people by automatic blood analyzer system. Hemoglobin zones above the cases were detected by electrophoresis, and erythrocyte fragility were detected by simple-tube test, HbA2 were detected by chromatography, HbF were detected by anti-base test, and erythrocyte morphology stained with Wright were analyzed under microscope respectively. Results The results of MCV and MCH, erythrocyte fragility test, HbA2 and HbF showed significantly different between the minor thalassemia group and the control group. Conclusions The parameters above blood samples are reliable for screening minor thalassemia. Thalassemia genes should be detected for the borderline cases with both normal hemoglobin and increased red blood cells.
