老年精神分裂症患者血浆D-二聚体水平及影响因素

目的对老年精神分裂症患者血浆D-二聚体(D-D)水平进行对照研究,同时分析其影响因素。方法测定老年精神分裂症患者(研究组,n=97)血浆D-D水平,与年龄、性别匹配的非分裂症老年人(对照组,n=97)进行对照研究,对年龄、治疗药物等与血浆D-D水平进行探索性分析。结果①研究组血浆D-D水平显著高于对照组,差异有统计学意义(t=8.320,P=0.000),协方差分析调整年龄、BMI影响后差异仍有统计学意义(F=6.425,P=0.016);②使用非典型抗精神病药患者血浆D-D水平显著高于使用典型抗精神病药患者(t=2.981,P=0.004);③研究组D-D异常增高(1000 mg/L)的发生率为19.6%(19/97例),显著高于对照组的8.2%(8/97例),差异有统计学意义(χ2=5.206,P=0.023);④D-D与年龄、BMI正相关(分别r=0.364,0.304;P=0.009、0.037)。结论老年精神分裂症患者、尤其是使用非典型抗精神病药患者血浆D-二聚体水平显著增高,提示其存在血液高凝状态。

Plasma D-dimer in Elderly Schizophrenia and Influencing Factors

Objective To investigate the plasma D -dimer level in elderly schizophrenic patients and its influencing factors .Meth-ods Levels of plasma D-dimer（D-D） was assessed in 97 cases of elderly with schizophrenia （research group） or without schizophre-nia（control group） matched with age and gender .Results Elderly schizophrenia in the research group had significantly higher plasma D-dimer level than those in the control group （t=8.320,P〈0.001）.In an analysis of covariance,the difference was still significant after adjusted by age,BMI（F=6.425,P〈0.016）.Schizophrenia with atypical antipsychotics treatment had significantly higher plasma D-dimer level than those treated with typical antipsychotics （t=2.981,P〈0.01） in research group.The ratio of D-D abnormal in-creased patients（ 〉1000 mg/L） was higher（χ2 =5.206,P=0.023）in research group（19.6%,19/97cases） than that in control group （8.2%,8/97cases）.The plasma level of D-D was significantly correlated with age （rs=0.364,P〈0.01） and BMI（rs=0.304,P〈0.05）.Conclusion Elderly schizophrenic patients ,especially those treated with atypical neuroleptic have increased D -D levels,which suggest higher coagulation status in them .
