原发性高血压患者IGF-1、CRP与颈动脉IMT的相关性

目的研究原发性高血压病患者血清胰岛素样生长因子-1（IGF-1）和C反应蛋白（CRP）水平与颈动脉内膜中层厚度（IMT）的相关性。方法高血压病患者65例，健康查体者（正常对照组）60例。用放射免疫法检测血清IGF-1水平，免疫散射比浊法测定血清中CRP含量，高分辨超声测定IMT作为大血管动脉粥样硬化（AS）程度的指标，分析IGF-1、CRP与1MT之间的关系。结果与正常对照组相比，EH组的血清IGF-1、CRP水平均显著增高（P〈O．01），IMT显著增厚（P〈0．01）。随着高血压分级水平的增高，IMT逐步增厚，颈动脉粥样硬化的程度逐渐加重，IGF-1和CRP浓度也增大。相关分析显示，血清IGF-1、CRP与IMT呈正相关（r=0．227，0．318；P〈0．05，P〈0．05）。结论血清IGF-1、CRP水平与EH患者IMT正相关，是EH患者]MT增厚的危险因素，提示IGF-1、CRP参与了EH患者AS病理过程，且其水平的高低与AS严重程度有关，可作为EH患者并发AS的早期预测及监测指标。

Correlation of levels of serum IGF-1 and CRP with carotid intima-media thickness in patients with essential hypertension

Aim To investigate the correlation of levels of insulin-like growth factor-1 （IGF-1） and C-reactive protein（CRP） with intima-media thickness （IMT） in patients with essential hypertension （EH）. Methods Sixty five EH cases and sixty sex-matched healthy control subjects were enrolled. The levels of serum IGF-1 and CRP were detected by RIA and immune scattering nephelometry.The IMT was detected by high-resolution ultrasound whch as a index of large vessels atherosclerosis. Results The levels of serum IGF-I and CRP in EH group were signilqcanfly higher than those in control group （P〈0.01）. With the increased level of blood pressure classifieation,IMT progressively thicker,and carotid atherosclerosis extent gradually increased,IGF-1 and CRP increased significantly. IMT ≥0.85mm group compared with IMT〈0.85mm group,the levels of IGF-1 and CRP were significantly higher,showing significant diference （P〈0.05）; In patients with EH,the levels of serum IGF-1 and CRP were positively correlated with IMT （r=0.227,0.318;P〈0.05,P〈0.05. Conclusion The levels of serum IGF-1 and CRP were positively correlated with IMT in EH patients, suggesting that IGF-1 and CRP participate the pathological process of arteriosclerosis, and correlated with severity of arteriosclerosis, thus they could be early predicting and monitoring indicators for EH patients complicated with AS.
