196例肛门及肛管尖锐湿疣病变中HPV感染的研究

摘    要:目的 探讨南京地区肛门及肛管尖锐湿疣病变中人乳头瘤病毒(HPV)不同基因型别的感染情况及其临床意义.方法 从196例肛门及肛管尖锐湿疣(CA)石蜡组织标本中提取23种HPV DNA,采用基因扩增芯片杂交技术对其进行HPV基因型别的检测,并对患者进行临床病理资料分析.结果 196例肛门及肛管CA病变组织标本中检出HPV总阳性率为67.86%(133/196),其中单一型别的阳性率为55.61%(109/196);单一型别的感染中HPV6型阳性率为28.06%(55/196),是最主要的感染型别,其次为HPV11型,阳性率为27.04%(53/196);混合型HPV感染阳性率为12.25%(24/196),其中HPV6型与11型混合感染占50.00%(12/24),是混合型感染的主要型别,此外还有5例4种型别的混合感染,占混合感染的20.83%(5/24).结论 基因扩增芯片检测技术是一种比较适合临床进行HPV分型检测的、敏感性高、特异性好的诊断方法,尤其适合开展某种病变中HPV感染的分子流行病学的研究.

Abstract:Objective To study the genotypes of human papillomavirus(HPV) and its clinical characteristics in condyloma acuminata(CA) of anus and anal canal in Nanjing city.Methods 23types of HPV genotype were detected in196patients with CA of anus and anal canal by polymerase chain reaction(PCR) and gene-chips technique,and the related clinical pathology data of patients were analyzed.Results In 196patients with CA of anus and anal canal,the total HPV infection rate was 67.86%(133 /196),with single type infection rate of 55.61%(109 /196).The predominant type of single infection with HPV was HPV6(28.06%,55 /196),followed by HPV11(27.04%,53 /196).The mixed infection rate was 12.25%(24 /196),and the predominant type of mixed infection was HPV6+11(50.00%,12 /24),followed by HPV6+11+X+X(20.83%,5 /24).Conclusion Gene-chips method could be a high sensitive and specific method in detection and typing of HPV,especially fit for study on molecular epidemiology of HPV infection.
