肝外胆管癌中Bcl-2、Bax和Fas的表达

目的研究Bcl-2、Bax、Fas在肝外胆管癌中的表达及临床意义。方法应用免疫组化SP法测定41例肝外胆管癌中Bcl-2、Bax、Fas的表达。结果（1）肝外胆管癌组Bcl-2阳性率为58．54％（24／41），其中高中分化癌组（A组）、低分化癌组（B组）、无转移组（C组）、转移组（D组）阳性率分别为75．00％（21／28）、23．08％（3／13）、76．00％（19／25）、31．25％（5／16）。A组与B组间、C组与D组间差异有显著性（P〈0．01）。（2）肝外胆管癌组Bax阳性率为56．10％（23／41），其中A组、B组、C组、D组阳性率分别为67．86％（19／28）、30．77％（4／13）、72．00％（18．25）、31．25％（5／16）。A组与B组间、C组与D组间差异有显著性（P〈0．05）。（3）肝外胆管癌组Fas阳性率为51．22％（21／41），其中A组、B组、C组、D组阳性分别率为67．86％（19／28）、15．37％（2／13）、76．00％（19／25）、12．50％（2／16）。A组与B组间、C组与D组间差异有显著性（P〈0．01）。结论Bcl-2、Bax、Fas的异常表达与肝外胆管癌的病理分级及转移有关，可作为判断胆管癌患者预后的参考指标。

The expression and clinical significance of Bcl-2,Bax,Fas in extrohepatic cholangiocarcinoma

Objective To study the expression and significance of Bcl-2,Bax,Fas in extrohepatic cholangiocarcinoma(ECHC).Methods The expression of Bcl-2,Bax,Fas in 41 cases of ECHC was detected by SP immunohistochemical staining.Results(1)The positive rates of Bcl-2 in well and (mediated-)differentiated group(A),poordifferentiated group(B),nonmetastasis group(C) and metastasis group(D) were 75.00%,23.08%,76.00% and 31.25% respectively.The expression rate of Bcl-2 was significantly related to histopathological grade and metastasis(P<0.01).(2)The positive rates of Bax in group A,group B,group C and group D were 67.86%,30.77%,72.00% and 31.25% respectively.The expression rate of Bax was associated with histopathological grade and metastasis((P<0.05).)(3)The positive rates of Fas in group A,group B,group C and group D were 67.86%,15.38%,76.00% and 12.50% respectively.The expression rate of Fas was associated with histopathological grade and metastasis(P<0.01).Conclusion The abnormal expression of Bcl-2,Bax,Fas is correlated with histopathological grade and metastasis.The expression of the three genes can be used to predict the prognosis of ECHC.
