132株副溶血性弧菌毒力基因及耐药性分析

摘    要:目的了解2014年-2017年桐乡市副溶血性弧菌分离株毒力基因携带与耐药性情况,为临床更为有效用药提供数据上的支持。方法利用RT-PCR技术对其分离株进行毒力(tlh、tdh、trh)检测;采用纸片扩散法操作,测定其对22种抗生素的耐药性。结果在132株副溶血性弧菌中,毒力基因tlh、tdh和trh阳性携带率分别为100.00%(132/132)、57.58%(76/132)和0.00%(0/132);对氨苄西林、头孢唑啉、哌拉西林、复方新诺明的耐药率分别为31.5%、23.5%、12.5%、12.5%,对除碳青霉烯类和喹诺酮类外的6大类抗生素均有耐药性,而其中对β-内酰胺类抗生素和叶酸代谢途径抑制剂及氯霉素的耐药性相对较高。多重耐药方面,对2种以上抗生素耐药的有15株,占11.90%。结论126株副溶血性弧菌分离株主要携带毒力基因为tlh、tdh。除氨苄西林等9类抗生素外,其余13类在副溶血性弧菌感染的治疗中可作为首选。

Abstract:Objective To analyze the virulence genes and drug resistance in Vibrio parahaemolyticus strains isolated in Tongxiang City from 2014 to 2017 in order to provide reference for the prevention and treatment and the rational use of medicines.Methods Virulence genes of tlh,tdh and trh in Vibrio parahaemolyticus strains were detected by fluorescence quantitative PCR,and drug resistance of 22 antibiotics was tested by disk diffusion.Results Among 132 strains of Vibrio parahaemolyticus,the positive rates of virulence genes tlh,tdh and trh were 100.00%(132/132),57.58%(76/132)and 0.00%(0/132),respectively.The drug resistance rates to ampicillin,cefazolin,piperacillin and compound trimethoprim were 31.5%,23.5%,12.5%and 12.5%,respectively.Except carbopenems and quinolones,all the Vibrio parahaemolyticus isolates were resistant to other kinds of antibiotics,showing relatively higher resistance toβ-lactams,folate pathway inhibitors and chloromycetins.And there were 15 isolates simultaneously resistant to more than 2 kinds of antibiotics(11.90%).Conclusion 126 isolates of Vibrio parahaemolyticus mainly carried virulence genes tlh and tdh.Except ampicillin and other 9 antibiotics,the remaining 13 antibiotics can be the first choice in the treatment of Vibrio parahaemolyticus infection.
