血清半胱氨酸蛋白酶抑制剂C早期诊断急性胰腺炎肾功能损伤的参考价值

目的分析血清半胱氨酸蛋白酶抑制剂C(Cys-C)对判别急性胰腺炎肾功能损伤的参考价值。方法选取2016年7月-2018年7月的284例急性胰腺炎患者为研究对象。测定血清Cys-C等指标后进行随访,根据血清Cys-C水平把研究对象分为4组,分别为Q1:Cys-C≤1. 06 mg/L;Q2:1. 07 mg/L^1. 48 mg/L;Q3:1. 49 mg/L^1. 85 mg/L;Q4:≥1. 86 mg/L。分析血清Cys-C、SCr、CCr、e GRF对急性胰腺炎患者肾功能损害的参考价值。结果 4组研究对象在性别、年龄、BMI、吸烟、饮酒、糖尿病、高血压上差异均无统计学意义(P> 0. 05);4组在血清肌酐水平、e GFR水平、Scr上差异均有统计学意义(P <0. 05);Cys-C的诊断灵敏度和特异度均显著高于SCr、CCr、e GRF(P <0. 05)。结论血清Cys-C对判别急性心力衰竭患者的预后具有较好的参考价值,值得临床上推广使用。

Reference value of early detection of renal function damage in acute pancreatitis by serum cystatin C

Objective To analyze the reference value of serum cystatin C( Cys-C) for discriminating renal function damage in acute pancreatitis. Methods A total of 284 patients with acute pancreatitis were enrolled from July 2016 to July 2018. The serum Cys-C and other indicators were measured and followed up. According to the serum Cys-C level,the subjects were divided into four groups,namely Q1: Cys-C ≤1. 06 mg/L;Q2: 1. 07 mg/L-1. 48 mg/L;Q3: 1. 49 mg/L-1. 85 mg/L;Q4: ≥1. 86 mg/L. The reference value of serum Cys-C,SCr,CCr and e GRF was analyzed for renal dysfunction in patients with acute pancreatitis. Results There was no statistical significance on the differences in gender,age,BMI,smoking,drinking,diabetes and hypertension among the four groups( P > 0. 05). There was statistical significance on the differences in serum creatinine,e GFR and Scr among the four groups( P < 0. 05);the diagnostic sensitivity and specificity of Cys-C were significantly higher than those of SCr,CCr and e GRF( P < 0. 05). Conclusion Serum Cys-C has a good reference value for judging the prognosis of patients with acute heart failure,and it is worthy of clinical promotion.
