儿童斑秃外周血Th17及Treg细胞相关细胞因子水平的变化及意义

摘    要:目的探讨儿童斑秃患者外周血Th17及Treg细胞相关细胞因子水平的变化及意义,从免疫学角度分析T淋巴细胞在儿童斑秃发病机制中发挥的作用。方法采用酶联免疫吸附试验(ELISA)检测50例儿童斑秃患者血清中Th17及Treg细胞相关细胞因子(IL-17、IL-6、TGF-β)水平,并与同期50例健康体检儿童进行比较。结果观察组外周血IL-17、IL-6水平显著高于对照组(P<0.05),TGF-β水平显著低于对照组(P<0.05);观察组中,不同脱发面积、病程长短、病情活动与否及是否初发间IL-17、IL-6、TGF-β水平差异均有统计学意义(P<0.05);观察组IL-17与IL-6水平呈显著正相关(r=0.760,P<0.001),而IL-17、IL-6与TGF-β水平均呈显著负相关(r值分别为-0.659、-0.642,P<0.001)。结论儿童斑秃的发病可能与Th17及Treg细胞相关细胞因子表达水平异常有关,其中Th17细胞相关细胞因子表达亢进,Treg细胞相关细胞因子表达下降。

Abstract:Objective To detect Th17 and Treg related cytokines in the children with alopecia areata,and to explore their role in the pathogenesis of immunology.Methods The levels of Th17 and Treg related cytokines(IL-17,IL-6,TGF-β)in 50 children with alopecia areata were measured by ELISA and compared with 50 healthy controls.Results The levels of IL-17 and IL-6 in the patients were higher than those in controls(P<0.05),and the levels of TGF-βwere lower than those in controls(P<0.05).In the children with alopecia areata,there were differences in the levels of IL-17,IL-6,and TGF-βbetween the hair loss area,primary or not,disease duration and activity(P<0.05).The levels of IL-17 and IL-6 were both negatively correlated with TGF-β(r=0.760,P<0.001),and IL-17 and IL-6 were positively correlated to each other(r=-0.659,r=-0.642,P<0.001).Conclusion The pathogenesis of alopecia areata in children may be related to the imbalance of Th17 and Treg related cytokines,which is manifested as hyper response of Th17 related cytokines and hypo response of Treg related cytokines.
