急性白血病患者免疫指标、凝血指标、基质衍生因子-1α及乳酸脱氢酶的检测及其临床意义

目的探究急性白血病患者免疫指标、凝血指标、基质衍生因子-1α(SDF-1α)、乳酸脱氢酶(LDH)的检测及其临床意义。方法选择2014年3月-2017年9月本院收治的急性白血病患者112例,其中初诊未治疗50例(初诊组),诱导治疗后完全缓解62例(完全缓解组),同期另选80例健康人作为对照组。对3组患者外周血免疫指标、凝血指标、SDF-1α及LDH进行检测并分析其临床意义。结果初诊组及完全缓解组CD4^+、CD4^+/CD8^+、NK、PLT、MPV水平均显著低于对照组,差异有统计学意义(P<0.05),而其CD8^+、D二聚体、SDF-1α、LDH水平显著高于对照组,差异有统计学意义(P<0.05);初诊组CD4^+、CD4^+/CD8^+、NK、PLT、MPV水平显著低于完全缓解组,差异有统计学意义(P<0.05),而其CD8^+、D二聚体、SDF-1α、LDH水平显著高于完全缓解组,差异有统计学意义(P<0.05)。结论急性白血病患者免疫功能较为异常、凝血功能及相关因子表达紊乱,临床上动态观察患者上述因子水平,对患者的恢复可能有一定积极意义。

Detection and clinical significance of immune index,coagulation index,SDF-1a and LDH levels in patients with acute leukemia

Objective To explore the detection and clinical significance of immune index,coagulation index,SDF-1 a and LDH levels in patients with acute leukemia.Methods From March 2014 to September 2017,112 patients with acute leukemia in our hospital were selected,among them 50 cases without treatment were enrolled as initial diagnosis group,62 cases were complete remission after induction therapy(complete remission group),and another 80 healthy people as the control group.Peripheral blood immune index,coagulation index,SDF-1 a and LDH in three groups were detected and their clinical significance was analyzed.Results The levels of CD4^+,CD4^+/CD8^+,NK,PLT and MPV in the initial diagnosis group and complete remission group were significantly lower than those in the control group,with the differences statistically significant(P<0.05),while the levels of CD8^+,D dimer,SDF-1α and LDH were significantly higher than those in the control group,with the differences statistically significant(P<0.05);the levels of CD4^+,CD4^+/CD8^+,NK,PLT and MPV in the initial diagnosis group were significantly lower than those in the complete remission group,with the differences statistically significant(P<0.05),while the levels of CD8^+,D dimer,SDF-1αand LDH were significantly higher than those in the complete remission group,with the differences statistically significant(P<0.05).Conclusion Patients with acute leukemia have abnormal immune function and abnormal expression of coagulation and related factors.Dynamic observation of the levels of these factors may be of positive significance to the recovery of patients.
