尿液肝素结合蛋白检测在尿路感染中的诊断价值

目的探讨尿液肝素结合蛋白(U-HPB)水平检测在尿路感染(UTI)中的诊断价值及临床意义。方法2016年1月-2018年6月本院收治的158例患者,包括89例UTI患者、24例疑似UTI患者、45例非UTI患者,另有同期50例健康体检者作为对照组,检测各组尿液HPB、白细胞介素-6(IL-6)、尿常规和尿有形成份,并对检测结果统计分析。结果UTI组U-HPB水平明显高于其他组,差异有统计学意义(P<0.05)。U-HPB、U-IL-6、尿液白细胞酯酶(U-LE)、尿液亚硝酸盐(U-NIT)和尿液白细胞(U-WBC)计数的曲线下面积(AUC)分别为0.917、0.798、0.670、0.743和0.852。以32.0 ng/ml为界值U-HPB诊断UTI的灵敏度和特异度分别为92.1%和88.6%。结论U-HBP能有效区分UTI和疑似UTI患者组,U-HPB检测具有简便、快速、敏感性、特异性,可作为尿路感染的诊断指标。

Clinical value of urine heparin-binding protein in the diagnosis of urinary tract infection

Objective To evaluate the significance of urine heparin-binding protein(U-HBP)in the diagnosis of urinary tract infection(UTI).Methods From January 2016 to June 2018,158 cases of patients included 89 UTI patients(UTI group),24 suspicious UTI patients(suspicious UTI group)and 45 non UTI patients(no UTI group),and 50 normal control(control group).Urine samples of all patients and 50 healthy subjects were collected,and urine HPB,interleukin-6(IL-6),urine routine and urine composition were detected in each group.The results of each group were statistically analyzed.Results U-HPB levels in the UTI group were significantly higher than those in the other groups,and the differences were statistically significant(P<0.05).The area under the curve(AUC)of U-HPB,U-IL-6,urine leukocyte esterase(U-LE),urine nitrite(U-NIT),and urine leukocyte(U-WBC)counts were 0.917,0.798,0.670,0.743,and 0.852.The sensitivity and specificity of U-HPB for UTI diagnosis with 32.0 ng/ml as the cutoff were 92.1%and 88.6%,respectively.Conclusion U-HBP can distinguish the UTI group from suspicious UTI group,the detection of U-HBP has good sensitivity and specificity,and the detection is also relatively convenient and fast,which can be used as an assistant index for the diagnosis of urinary tract infection.
