外周血T淋巴细胞亚群和细胞因子联合检测在早期识别嗜血细胞综合征中的应用价值

摘    要:目的探讨外周血T淋巴细胞亚群和细胞因子联合检测在早期识别嗜血细胞综合征(HPS)中的应用价值。方法回顾性分析2015年12月-2018年12月本院确诊嗜血细胞综合征为HPS组(30例),对照组为同期因发热住院非HPS患儿30例,健康组取同期体检的健康儿童30例。比较3组CD4^+和CD8^+T淋巴细胞亚群、γ-干扰素(IFN-γ)和白介素-2(IL-2)水平。结果3组IFN-γ与IL-2水平比较,差异有统计学意义(F值分别为32.97、291.49,P<0.01)。HPS不同病程IFN-γ与IL-2水平比较,差异有统计学意义(F值分别为10.91、4.14,P<0.05);3组CD4^+与CD8^+T淋巴细胞亚群水平比较,差异有统计学意义(F值分别为60.36、10.23,P<0.01);HPS不同病程CD4^+与CD8^+T淋巴细胞亚群水平比较,差异有统计学意义(F值分别为13.38、7.03,P<0.01);HPS与IFN-γ和IL-2水平呈显著相关(r值分别为0.73、-0.66,P<0.01),而与CD4^+和CD8^+T淋巴细胞亚群亦呈显著相关(r值分别为0.74、-0.57,P<0.01)。结论外周血IFN-γ和IL-2水平联合CD4^+和CD8^+T淋巴细胞亚群检测有助于早期识别嗜血细胞综合征。

Abstract:Objective To observe the combined detection value of T lymphocyte subsets and cytokines in peripheral blood in the early identification of hemophagocytic syndrome.Methods Children diagnosed with hemophagocytic syndrome were selected as the HPS Group,30 non-HPS children hospitalized for fever in the same period were selected as the control group and 30 healthy children in the same period were taken as the control group,then the levels of CD4^+and CD8^+T lymphocyte subsets,IFN-γ,and interleukin-2(IL-2)levels were compared in the three groups.Results The levels of IFN-γand IL-2 in peripheral blood were compared,and the differences were statistically significant among the 3 groups(F=32.97,F=291.49,P<0.01).Then the levels of IFN-γand IL-2 in different course of hemophagocytic syndrome in peripheral blood were compared,and the differences were statistically significant among the 3 groups(F=10.91,F=4.14,P<0.05);the levels of CD4^+and CD8^+T lymphocyte subsets in peripheral blood were compared,and the differences were statistically significant among the 3 groups(F=60.36,F=10.23,P<0.01).Then the levels of CD4^+and CD8^+T lymphocyte subsets in different course of hemophagocytic syndrome in peripheral blood were compared,and the differences were statistically significant among the 3 groups(F=13.38,F=7.03,P<0.01).Hemophagocytic syndrome was significantly correlated with IFN-gamma and IL-2 levels(r=0.73,r=-0.66,P<0.01),and which was also correlated with CD4^+and CD8^+T lymphocyte subsets(r=0.74,r=-0.57,P<0.01).Conclusion It is helpful to early identification of hemophagocytic syndrome by the Detection of IFN-γand IL-2 levels in combination with CD4^+and CD8^+T lymphocyte subsets in peripheral blood.
