妊娠期糖尿病的高危因素及对妊娠结局的影响

目的研究孕妇妊娠期糖尿病(GDM)发病的高危因素及与不良妊娠结局的关系,为优生优育提供理论依据。方法选取2016年1月-2018年6月在本院孕期体检的1 642例单胎妊娠孕妇作为研究对象,回顾性分析其临床资料,采用卡方检验和Logistic多因素分析筛选GDM发病的高危因素及分析GDM患者的妊娠结局。结果 1 642例孕妇中检出62例GDM患者,检出率为3.78%。单因素和进一步Logistic回归多因素分析显示,年龄≥35岁、糖尿病家族史、BMI≥30 kg/m^2、CRP≥8 mg/L和孕期增重≥15 kg是孕妇发生GDM的独立危险因素(P<0.05)。GDM组孕妇出现不良妊娠结局的发生率明显高于对照组(P<0.01)。结论妊娠期糖尿病的高危因素较多,且严重影响妊娠结局,临床上采取针对措施预防妊娠期糖尿病发病,对优生优育有重要意义。

High risk factors for gestational diabetes mellitus and their impact on pregnancy outcomes

Objective To study the high risk factors of gestational diabetes mellitus in pregnant women and its relationship with adverse pregnancy outcomes,so as to provide theoretical basis for prenatal and postnatal care.Methods A total of 1 642 pregnant women with singleton pregnancy who were examined in our hospital during January 2016-June 2018 were selected as the study subjects.The clinical data was retrospectively analyzed.The risk factors of GDM were screened by chi-square test and Logistic multivariate analysis.The pregnancy outcomes of the two groups of patients were compared.Results Of the 1 642 pregnant women,62 patients with GDM were detected,and the detection rate was 3.78%.Univariate and further Logistic regression multivariate analysis showed that age≥35,family history of diabetes,BMI ≥ 30 kg/m^2,CRP ≥ 8 mg/L and pregnancy weight gain≥15 kg were independent risk factors for GDM in pregnant women(P<0.05).The incidence of adverse pregnancy outcomes was significantly higher in the GDM group than that in the control group(P<0.01).Conclusion There are many high risk factors for gestational diabetes,which seriously affect the pregnancy outcome.It is necessary to take measures to prevent the onset of gestational diabetes,which is of great significance for prenatal and postnatal care.
