肝癌及癌旁组织中HSP70和bcl-2表达及意义

摘    要:目的 探讨肝癌及癌旁组织中HSP70和bcl-2表达及意义。方法 应用免疫组织化学Elivision法检测58例肝细胞癌及癌旁组织HSP70和bcl-2表达,并作HE染色,进行肝癌组织学分级(Ⅰ-Ⅳ级,Edmondson-Kondo标准)。结果 58例肝癌中Ⅰ级7例,Ⅱ级28例,Ⅲ级18例。Ⅳ级5例,癌旁有肝硬化改变者47例(81.0%);HSP70阳性反应产物呈棕黄色颗粒状,主要定位于胞质,部分细胞同时出现核及胞膜着色。癌组织HSP70阳性率(70.7%)及阳性强度明显高于癌旁肝组织(37.9%),肝癌分化越差,HSP70表达越强。另外,癌旁肝硬化组织中增生的小胆管上皮细胞内HSP70呈普遍中度阳性表达;癌旁肝组织的bcl-2阳性率为77.6%,呈簇状和/或片状分布,癌组织中bcl-2阳性率为19.0%,且阳性细胞数明显少于癌旁组织,多呈碎点状。结论 HSP70表达水平可反映肝癌细胞的增殖及恶性程度,有一定诊断和预后价值;肝癌和癌旁组织bcl-2染色对比显示肝癌细胞发生发展过程中抑凋亡机制减弱;HSP70可能参与肝癌及癌旁细胞的凋亡调控。

Abstract:[Objective] To investigate the expression and relationship of HSP70 and bcl-2 proteins in the primary hepatocellular carcinoma (HCC) and their paracarcinomatous tissues. [Methods] The expression of HSP70 and bcl-2 proteins was evaluated in paraffin-embedded tissue by immunohistochemical techniques (Elivision) in 58 cases of hepatocellular carcinoma. [Results] The positive expression of HSP70 displayed as brown particles was mainly located in the cytoplasm, and was also coexisted partly in the nucleus and membranes. The positive rate of HSP70 in HCC was much higher than that in paracarcinomatous tissues (70.7% vs. 37.9%). The expression of HSP70 in HCC was related to the histological grading, the lower the differentiation of HCC, the higher the expression of HSP70. An unexpected finding was that HSP70 expression was prevailed in the bile duct cells in the paracarcinomatous tissues. The positive rates of bcl-2 in cancer and paracarcinomatous tissues were 19.0% and 77.6% respectively. The bcl-2 expression was located in cytoplasm and its positive signals were strong and clustery in paracarcinomatous tissues, while the number of the bcl-2 positive cell was sporadic and the signals were weak and "piece-meal like" in HCC. [Conclusions] The results show that the expression of HSP70 in HCC may have diagnostic and prognostic value on account of its reflecting the proliferation and the malignant degree of HCC. The different expression of bcl-2 in HCC and peri-HCC suggests that the anti-apoptotic mechanism was abated in the liver cell carcinogenesis. The expression of HSP70, which was in inverse proportion to the bcl-2 staining, may play a role in the apoptosis of HCC and paracarcinomatous tissues.
