急性脑梗死患者血浆D-二聚体动态变化与预后关系研究

目的　探讨血浆D -二聚体在急性脑梗死 (ACI)发病过程中的变化和预后关系。方法　采用Latex半定量法动态检测 93例患者及 2 0名正常对照者血浆D 二聚体变化 ,分析其与梗死面积大小 ,病情和预后的相关性。结果　ACI患者血浆D 二聚体水平与病情及预后关系密切 ,其中大梗死组血浆D 二聚体升高水平高 ,持续时间长 ,神经功能缺损程度越重 ,预后越差 ,与中、小梗死组比较 ,差异显著 (P <0 .0 1)。结论　ACI患者血浆D 二聚体水平可作为判断病情及预后的客观指标 ,D 二聚体越高 ,持续时间越长 ,预后越差。

The relationship between the changes of plasma D-dimer and prognosis in patients with acute cerebral infarction

Objective To observe the relationship between the changes of plasma D dimer in pathogenic course and prognosis in patients with acute cerebral infarction(ACI). Methods Changes in plasma D dimer levels of 93 patients with ACI and 20 cases healthy persons were detected dynamically by Latex semi quantitative method. The relation between D dimer levels and focus size, severity of infarction and prognosis were analyzed. Results There was significant difference( P <0.01) between the level of D dimer in patients with ACI and that in control group. The larger the infarction size was, the higher the D dimer was( P <0.01). D dimer in severe group was higher than that in mild group ( P <0.01). A negative correlation between plasma D dimer and neurological functional recovery degree was found. Conclusion The plasma D dimer level in patients with ACI can be regarded as the objective index to judge patients' condition and prognosis. The higher the D dimer level is and the longer it lasts, the poorer the prognosis is.
