大隐静脉-隐神经营养血管皮瓣修复下肢软组织缺损

目的探讨大隐静脉．隐神经营养血管皮瓣修复软组织缺损的手术方法及临床应用。方法分别顺行、逆行切取大隐静脉．隐神经营养血管皮瓣53例，转移修复膝关节、小腿、足踝部等皮肤软组织缺损创面。结果53例术后6例皮瓣肿胀。6例皮瓣远端的边缘部分坏死，范围小于3cm×1cm，经过处理后53例皮瓣全部成活，均随访8～24个月，皮瓣质地柔软，外观满意，膝、踝关节功能恢复良好，皮肤感觉有一定程度恢复。结论大隐静脉．隐神经营养血管皮瓣不需吻合血管，皮瓣设计灵活，操作简便，血供可靠，成活率高，是修复膝关节、小腿、足踝部等皮肤软组织缺损的较好方法。

Surgical Method and Clinical Application of Soft Tissue Defect Repair of Lower Limbs with Great Saphenous Vein-saphenous Nerve Nutritional Vessels Flap

Objective To explore the surgical method and clinical application of soft tissue defect repair with great saphenous vein -saphenous nerve nutritional vessels flapr of. Methods 53 cases of saphenous nerve nutritional vessels flap weretransferred to repair skin and soft tissue defect wound, such as knee, calf, ankle department etc. Results In all 53 cases, swelling of postoperative skin flap was observed in 6 cases, 6 cases appeared skin flap necrosis in edges on the far side , and the range was less than 3 cmx 1 cm. All skin flap survived, the appearance was satisfactable, and the function of knee and ankle joint were recoveried well after 24-month follow-up. Conclusion Great saphenous vein -saphenous nerve nutritional vessels flap is a good method to repair skin and soft tissue defect with reliable blood supply andhigh survival rate .
