肥胖儿童白细胞计数与糖脂代谢及心血管危险因素的关系

摘    要:目的:探讨肥胖儿童白细胞计数(WBCC)与糖脂代谢及心血管危险因素的关系。方法:收集我院儿科内分泌门诊8~15岁肥胖儿童111例,其中男84例,女27例。将WBCC分为WBCC1、WBCC2、WBCC3和WBCC4共4层,分别比较男孩、女孩各组糖脂代谢指标。对年龄及体质量指数(BMI)进行校正后采用偏相关分析不同性别儿童WBCC与糖脂代谢及心血管危险因素的关系,多元逐步回归分析影响WBCC水平的因素。结果:男孩WBCC、BMI、腰围(WC)及三酰甘油(TG)高于女孩,而收缩压(SBP)、舒张压(DBP)、空腹血糖(FPG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)、HOMA-IR及致动脉粥样硬化指数(AI)差异无统计学意义(P>0.05)。男孩及女孩的WBCC4组WC及BMI均高于WBBC1组,男孩WBCC4组TG及AI高于WBCC1组,WBCC4组的HDL-C低于WBCC1、WBCC2组(P<0.05)。男孩WBCC与BMI、WC、TG及AI呈正相关(P<0.05);女孩WBCC与BMI及WC呈正相关(P<0.05)。TG、WC、HDL-C为影响WBCC的因素。结论:WBCC与肥胖儿童中心性肥胖、血脂异常及糖代谢紊乱关系密切,早期检测WBCC水平并给予早期干预,可成为防止心血管疾病发生的简单、有效措施。
