中心静脉导管不同固定方法的对照研究

目的 探讨螺旋缠绕法固定中心静脉导管在临床中的应用.方法 对436例胸部手术患者行中心静脉置管,将选择对象随机分为对照组和实验组两组,对照组采用常规方法固定,实验组采用螺旋缠绕法固定,穿刺点采用带小棉垫的透明贴膜覆盖,比较两组固定方法的效果.结果 实验组患者导管局部红肿、不适感、导管成角、导管部分脱出及导管脱落的发生率均明显少于对照组,两者比较有统计学意义(P＜0.05);加用带小棉垫的透明贴膜覆盖穿刺点可降低局部感染的发生率,减少导管自穿刺点部分脱出及导管脱落的发生率.结论 螺旋缠绕法固定中心静脉导管在临床应用中具有明显的优点,采用带小棉垫的透明贴膜覆盖穿刺点可降低局部感染率,减少导管脱出的发生率.

A Comparison Study of Different Fixation Methods for Central Venous Catheter

Objective To study the application of spiral fixation for central venous catheter in clinical nursing.Methods Total 436 patients receiving cardiothoracic surgery received central venous catheter and they were randomly divided into 2 groups,namely,the routinely fixed group and the spirally fixed group.The puncture sites were covered with transparent membrane and the outcomes were compared between the 2 groups.Results The experimental group had lower incidences of inflammation,angulations,part pull-through and full-slippage,but more comfortness than the routine group(P<0.05).The application of transparent dressings with a little cotton cushion decreased the incidences of local inflammation,part pull-through and full-slippage.Conclusion The spiral fixation of central venous catheter has great advantages for easy nursing.The application of transparent dressing with a little cotton can help to decrease local inflammation and full-slippage.
