高血压性脑出血早期病情恶化的相关因素分析

摘    要:目的探讨影响高血压性脑出血早期病情恶化的相关因素。方法回顾性分析64例高血压性脑出血患者早期症状恶化且在24h内行2次头颅CT检查的病历资料,并分为血肿扩大组及血肿稳定组,分析各组病情恶化原因。结果42例血肿扩大组经治疗后死亡18例,22例血肿稳定组经治疗后死亡5例。结论高血压性脑出血早期病情恶化死亡率高,导致早期病情恶化的因素主要是血肿扩大,血肿扩大与血压、出血部位、血肿形态等有关,血肿稳定但病情恶化与首次头颅CT血肿大小,出血部位、颅腔代偿能力等有关。

Abstract:Objective To investigate the correlative factors refered to early deterioration of hypertensive intracerebral hemorrhage.Methods Retrospective studies of clinical data had been done in 64 cases of hypertensive intracerebral hemorrhage with early symptoms deteriorate within 24 hours and cranial CT examination has been done twice.Patients divided into hematoma expansion group and hematoma stabilization group.Reasons of deterioration has been analized.Results 18 cases out of the 42 cases of hematoma expansive group died after treatment.5 cases out of 22 hematoma stabilized group died after treatment.Conclusion Mortality rate of hypertensive intracerebral hemorrhage in early deterioration cases is high.Main factors related to early deterioration were expansion of hematoma.The expansion of hematoma was related to blood pressure,location of bleeding and morphology of hematoma.Those patients whose hematoma stabilized but situation still deteriorated were related primary cranial CT,size of hematoma,location of hemorrhage and compensative ability of cranial cavity.
