直肠外翻切除结肠直肠(肛管)吻合术治疗低位直肠癌28例报告

目的:观察直肠外翻切除,结肠、直肠(肛管)吻合术治疗低位直肠癌的临床效果.方法:28例低位直肠癌采用外翻切除,经肛门外行结肠、直肠(肛管)吻合,并于肠腔内放一支撑管.术后观察并发症、排便功能、局部复发及生存情况.结果:术后无1例吻合口瘘发生,吻合口狭窄3例(10.7%),术后1个月均能预知和控制排便,术后半年每日排便1～3次26例,有2例偶有粪便外溢.术后3、5年局部复发率分别为5.26%、29.3%,3、5年生存率分别为94.43%、52.22%.结论:直肠外翻切除,结肠、直肠(肛管)吻合治疗低位直肠癌是可行的.

Everting Rectal Resection, Colorectal or Colon-anal Anastomosis in the Treatment of Lower Rectal Carcinoma: A Report of 28 Cases

Objective: To evaluate the clinical effect of everting rectal resection, colorectal or colon-anal anastomosis in the treatment of lower rectal carcinoma. Methods: Twenty-eight patients with lower rectal carcinoma received everting resection, colorectal or colon-anal anastomosis. Results: No anastomotic leakage occurred, anastomotic stricture in 3 cases (10.7%). All patients could sense stool and control defecation one month after operation, 26 cases with 1-3 bowel movements a day after half a year, only 2 cases with occasional overflow incontinence. Local recurrence rate at 3, 5 years was 5.26% and 29.3%,respectively. Survival rate at 3,5 years was 94.43% and 52.22%, respectively. Conclusion: Everting rectal resection, colorectal or colon-anal anastomosis is feasible for the treatment of lower rectal carcinoma.
