真菌性鼻窦炎的诊断鉴别诊断及手术治疗临床分析

目的:探讨真菌性鼻窦炎的诊断、鉴别诊断及手术治疗。方法:对40例经手术病理证实为真菌性鼻窦炎的临床表现、体征、影像学及手术治疗效果进行总结分析。结果:32例术前经临床表现,结合CT明确诊断,8例为术后经病理检查才明确诊断,40例均在鼻内镜下行鼻窦病灶清理术,术后用抗真菌药物冲洗鼻腔,全部病例均痊愈。结论:该病大多有典型的临床表现,CT有明确的特征性,手术彻底清除病灶,通畅引流,抗真菌药物冲洗鼻腔是有效的治疗方法。

The Diagnosis and Differentional Diagnosis and Surgical Treatment of Fungal Sinusitis

Objective:To discuss in the diagnosis,differentional diagnosis and surgical treatment of fungal sinusitis.Methods: In 40 cases of fungal sinusitis diagnosed by pathology,their clinical manifestations,imaging characteristics and surgical efficacy was summarized and analyzed.Results:Of 40 cases of fungal sinusitis,32 were diagnosed by clinical manifestations and CT examination before surgery,8 by pathological examination after surgery.All cases performed nasal sinus debridement and postoperative irrigation with antifungal agents.All cases were cured.Conclusion: Fungal sinusitis has typical clinical manifestations,charactistic CT imaging.Surgery can debride focus and drain smoothly.Antifungal agents for irrigating nasal cavity is the key in treatment of fungal sinusitis.
