氯氮平合并塞来昔布治疗慢性难治性精神分裂症随机双盲对照研究

目的探讨氯氮平合并塞来昔布治疗慢性难治性精神分裂症的疗效。方法符合美国精神障碍诊断与统计手册第四版诊断标准的28例慢性难治性精神分裂症患者，随机分配到研究组（氯氮平合并安慰剂）与对照组（氯氮平合并塞来昔布）。氯氮平平均剂量为（259±114）mg／d；塞来昔布第1周为200mg／d，以后为400mg／d。两组观察时间均为8周。以阳性和阴性症状量表（PANSS）、临床疗效总评量表（CGI—S）评定临床疗效，以汉密尔顿抑郁量表（HAMD）评定抑郁症状，以不良反应量表（TESS）、Simpson锥体外系不良反应量表（SEPS）、异常不自主运动量表（MMS）评定药物不良反应和锥体外系不良反应。结果第8周末与基线相比，两组PANSS总分及分量表均显著下降，差异有统计学意义（P〈0．05），而两组之间的PANSS总分、分量表减分率差异无统计学意义（P〉0．05）；两组不良反应发生率无显著性差异。结论免疫调节剂-塞来昔布不能提高氯氮平对慢性难治性精神分裂症的疗效。

Adjunctive celecoxib treatment with clozapine in chronic refractory schizophrenia:a double blind,randomized and placebo controlled trial

Objective To investigate the efficacy of adjunctive celecoxib treatment with elozapine in chronic refractory schizophrenia. Methods 28 chronic patients who satisfied the DSM - Ⅳ criteria for schizophrenia（ verified by the structured clinical interview for DSM） were involved. The patients were randomly assigned to either elozapine plus eeleeoxib group or clozapine plus placebo group, treated by 150 -450 mg/day of elozapine plus placebo or elozapine plus 400mg/day of eeleeoxib for 8 weeks. The efficacy and side effect were evaluated with PANSS, CGI - S, HAMD, TESS, SEPS and AIMS. Results At the end of 8th week, both groups of patients showed significant improvement in totle and subseales of PANSS. However, no significant differences were found in total scores and reduction rates of PANSS between two groups. Conclusion Adjunctive eeleeoxib treatment with elozapine in chronic schizophrenia did not improve the efficacy of elozapine.
