顺铂、卡铂治疗晚期乳腺癌随机对照研究

目的探讨顺铂(PDD)、卡铂(CBP)治疗晚期乳腺癌临床应用效果及不良反应。方法80例晚期乳腺癌随机分成NC组(40例)接受NC方案(盖诺+卡铂),NP组(40例)接受NP方案(盖诺+顺铂)治疗。结果NC、NP组近期疗效(CR+PR)分别为42.50%、37.50%,两组差异无显著性(P>0.05)。NC组患者生存质量明显好于NP组(P<0.05),NP组的Ⅲ度以上恶心、呕吐发生率比NC组高,NC组的Ⅲ~Ⅳ度骨髓抑制比NP组严重(P<0.05),两组生存率相似。结论NP方案和NC方案治疗晚期乳腺癌效果相近,生存率相似。但NC方案胃肠道反应轻,生存质量好,值得临床进一步推广。

A comparative study of DDP vs. CBP in treatment of advanced breast cancer

Objective To discuss the clinical effects and adverse reaction of DDP and CBP in treatment of advanced breast cancer.Methods 80 patients with advanced breast cancer were randomly allocated to two groups: NC group(n=40) received NC regimen(Navelbine plus CBP) and NP group(n=40) received NP regimen (Navelbine plus DDP).Results The short-term response rates(CR plus PR) of NC and NP groups were 42.50% and 37.50%,respectively,there was no statistical difference(P>0.05).The quality-of-life in NC group was better than NP group(P<0.05),the incidence of grade Ⅲ nausea and vomiting in NP group was higher than those in NC group,the incidence of leukopenia and thrombocytopenia in NC group was higher than those in NP group(P<0.05),the survival rate in the two groups was almost the same.Conclusion The response of NP regimen was similar to that of NC regimen in treatment of advanced breast cancer.Toxicity response was not exhaustedly.But the gastrointestinal tract response of NC regimen was low,and can improve the quality of life of patients.It was deserved clinical further observation,instead of NP regimen.
