骨髓基质细胞移植对心肌梗死大鼠左室功能的影响

摘    要:目的 :应用超声心动图评价骨髓基质细胞 (marrow stromal cell,MSC)移植对心肌梗死大鼠左室形态和收缩功能的影响。方法 :46只 SD大鼠通过结扎左冠状动脉前降支建立心肌梗死模型 ,随机分成局部移植组和冠状动脉移植组 ,每组再分成 5-氮杂胞嘧啶核苷 (5-aza)诱导组、未诱导组和对照组。分别在心肌梗死前、心肌梗死后 1周、移植后 2周、 4周、 8周行超声心动图检查 ,测量左室壁厚度 (AW、 PW)、左室舒张末期内径(L VDd)和左室射血分数 (EF)。结果 :MSC移植后 2周、 4周和 8周 ,移植组 L VDd均较对照组缩小 (P<0 .0 1) ;EF均高于对照组 (P<0 .0 5) ;局部移植和冠状动脉移植的 SD大鼠诱导组和未诱导组之间 EF无明显差异(P>0 .0 5) ;无论 MSC是否经过诱导 ,冠状动脉移植组效果均优于局部移植组 (P均 <0 .0 1)。结论 :自体 MSC移植可有效改善大鼠心肌梗死后 EF,影响左室重构 ,是治疗缺血性心脏病充血性心力衰竭的新方法

Abstract:Objective:Marrow stromal cell (MSC) t ra nsplantation is promising for the treatment of end-stage heart failure This st udy investigated the value of autologous MSC transplantation into infarcted myoc ardium through intramyocardial injection and intracoronary infusion Methods:Myocardial infarction was created in 46 rats by left anterio r desending coronary artery ligation All the rats were divided into two groups: intracoronary implantation group(IC) and intramyocardial implantation group(IM) Each group consisted of three subgroups:injection of induced MSC cells, MSC cells and culture medium alone(control rats) 7 days after infarction Left vent ricular size and function were assessed by echocardiography before and 1 week af ter infartion,2 weeks,4 weeks and 8 weeks after implantation The thickness of l eft ventricular wall,the diastolic diameter of left ventricle(LVDd) and EF were measured Results:LVDd of implantation group was smaller compared wi th that of the control group 2 weeks,4 weeks and 8 weeks after MSC implantation ( P <0 01)and EF was increased ( P <0 05) The improvement in EF was s imilar in the MSC group and the induced MSC group The improvement in EF was gre ater in the IC than that in IM( P <0 01) Conclusions:Autologous MSC transplantation improves contra ctibility of infracted heart This technique may lead to a novel therapy to trea t congestive heart failure caused by ischemic heart disease
