摘    要:目的:应用Accu-Check Performa血糖仪观察动脉、静脉、手指毛细血管血糖结果是否存在差异。方法:随机选择全麻下手术患者150例,其中男性84例,女性66例,年龄范围16~85岁;胰腺手术44例,胆道手术69例,胃肠手术16例,肺手术21例。150例均取右颈内静脉血、桡动脉或股动脉血和手指毛细血管血检查血糖。为避免单一消毒溶液对手指毛细血管血糖结果的影响,150例随机分成A、B 2组,每组75例。A组为动脉、静脉、手指采血前以75%酒精溶液消毒皮肤;B组为动脉、静脉、手指采血前以1%活力碘溶液消毒皮肤。手指采血前均以干棉棒吸干消毒溶液。以Accu-Check Performa血糖仪检测血糖,三者同时进行检测。结果:A、B 2组手指毛细血管血糖值均较同组静脉和动脉血糖值显著升高(P<0.001),而动脉血糖值又较静脉血糖值显著升高(P<0.001)。结论:150例临床观察表示,动脉、静脉、手指毛细血管血糖结果差异明显。临床工作中为准确观察患者血糖的动态变化,应用同一方法检测为宜。

Abstract:Objective To observe the difference of glucose concentration from arterial, vein and finger capillary blood. Mothods One hundred and fifty patients undergoing general anesthesia were selected randomly. Among them, 84 cases were male, 66 cases were female, with their ages ranging from 16 to 85 years old. All the blood samples were collected from right internal jugular vein and radial artery or femoral arterial at the same time, and finger capillary blood sample was also taken simultaneously. Blood glucose concentrations of the samples were checked. In order to avoid the effect of single disinfectant solution on blood glucose results, 150 patients were randomly divided into groups A and B. 75% ethanol solution and 1% povidone iodine were selected to disinfect the skin of patients in group A(n=75) and B(n=75) respectively. The disinfection solution was dried by using cotton stalks before the blood samples were collected. Bio-sensors-based blood glucose monitoring Accu-chek Advantage was used to detect blood glucose.Results The results taken on the same tendency either in group A or B: glucose concentration in finger capillary blood was higher than arterial blood's(P〈0.001), moreover, glucose concentration in arterial blood was higher than venous blood's(P〈0.001). Conclusion Clinical trial shows that glucose concentrations are very different in finger capillary blood, arterial blood and venous blood. The same method has to be used to collect blood samples in dynamic observation of blood glucose's change in clinical work.
