老年性2型糖尿病患者骨代谢指标分析

摘    要:目的:探讨老年性2型糖尿病患者骨密度(BMD)及骨生化指标的变化情况,阐明老年性2型糖尿病患者骨质疏松症相关危险因素。方法:测定110例男性2型糖尿病患者和87例正常男性的BMD,并检测生化及骨代谢指标。结果:老年性2型糖尿病患者组BMD降低的发生率明显高于对照组(P0.01),血清骨钙素(BGP),胰岛素(Ins)水平则显著低于对照组(P0.01),尿脱氧吡啶啉(Dpd),空腹血糖(FPG)高于对照组(P0.01)。结论:老年性2型糖尿病患者较正常人群易发生骨质疏松症,BMD、BGP、Dpd水平是诊断糖尿病性骨质疏松症(DOP)的重要指标,良好的血糖和血脂的调控及尽可能的保护胰岛β细胞可降低骨质疏松症的发生。

Abstract:Obejective:To investigate the changes of bone mineral density (BMD) and various metabolism biochemical profiles and to elucidate the relevant risk factors for osteoporosis in senile patients with type 2 diabetes mellitus (T2DM). Methods:Bone density, some biochemical indexes and bone metabolic parameters were detected in 110 male patients with T2DM and 87 normal controls. Results: The incidence rate of BMD reduction, urine deoxypyridinoline (Dpd) and fasting plasma glucose (FPG) in diabetic patients was significantly higher than those in control group (P0.01), while the serum level of bone gla-protein (BGP) and insulin (INS) in diabetic group were both significantly lower than that of control group (P0.01). Conclusion:Osteoporosis can easily occur in senile patient with T2DM. BMD, BGP and Dpd are important indexes to diagnose diabetic osteoporosis (DOP), while fine control of the levels of blood glucose and blood lipids aiming to protect pancreatic-cell may reduce the occurrence of osteoporosis.
