糖尿病肾病患者检测尿液微量白蛋白以及血清胱抑素C的临床价值

目的探讨尿液微量白蛋白（mALB）与血清胱押素C（CysC）联合检测对早期糖尿病肾病（DN）的诊断价值。方法采用免疫比浊法测定51例早期糖尿病肾痛患者（DN组）、35例无肾脏损伤的糖尿病患者（NDN组）、30例健康对照者（对照组）的尿液mALB、血清CysC水平。结果①DN组尿液mALB、血清CysC显著高于NDN组与对照组（P〈0．05），NDN组与对照组之间尿液mALB、血清CysC相比差异无统计学意义（P〉0．05）。②尿液mALB诊断DN的灵敏度、特异度分别为74．5％、95．4％，血清CysC诊断DN的灵敏度、特异度分别为80．4％、96．9％，尿液mALB联合血清CysC诊断DN的灵敏度、特异度分别为922％、923％．结论尿液mALB与血清CysC联合检测可大大提高诊断DN的灵敏度。

Clinical value of determination of urine microalbumin and serum cystatin C in patients with diabetic nephropathy

Objective To explore the clinical value of urine mieroMbumin （mALB） combined with serum eystatin C （CysC） in the diagnosis of early - stage diabetic nephropathy （DN）. Methods The urine mALB and serum CysC of 51 eases of early - stage DN（DN group）, 35 eases of non diabetic renal disease in diabetic pafients（NDN group） and 30 cases of healthy controls（control group） were determined by turbidimetric immunoassay. Results ①The urine mALB and serum CysC in DN group were significantly higher than those in NDN group and control group, respeetively（P 〈 0.05）, and there were no significant differences in the urine mALB and serum CysC between NDN group and control group. ②The urine mALB had a sensitivity of 74.5% ,specificity of 95.4% in the diagnosis of DN, and the serum CysC had a sensitivity of 80.4 %, specificity of 96.9 %, but combined detection of urine mALB and serum CysC had a sensitivity of 92.2 %, specificity of 92.3 %. Conclusion Combined detection of urine mALB and senma CysC can obviously improve the sensitivity in the diagnosis of DN.
