溃疡性结肠炎结肠粘膜固有层淋巴细胞亚型的分布特征及其作用

目的 探讨溃疡性结肠炎免疫学发病新观点。方法 应用抗T淋巴细胞（CD45RO）、抗B淋巴细胞（CD20）、抗T辅助淋巴细胞（OPD4）以及抗巨噬细胞（CD68）表面抗原的单克隆抗体，通过免疫组织化学技术，对50例溃疡性结肠炎患者结肠粘膜活检标本（32例活动期，18例非活动期）及5例对照组进行了淋巴细胞亚型分析。结果 T淋巴细胞及T辅助淋巴细胞的数量在溃疡性结肠炎活动期均高于非期及对照组（P＜0.01），而固有层内B淋巴细胞的数量在3组之间无显著性差异（P＞0.05）；巨噬细胞的数量在活动期高于非活动期及对照组（P＜0.01）。结论 T淋巴细胞尤其是T辅助淋巴细胞及巨噬细胞的活化与性结肠炎的活动密切相关，说明细胞免疫在溃疡性结肠炎免疫发病机制蝇占有相当的比重，参与溃疡性结肠炎活动期结肠粘膜的损伤，并已成为其免疫调节网络中重要的组成成分。

EFFECT AND DISTRIBUTIVE CHARACTERISTICS OF MUCOSAL LAMINA PROPRIA LYMPHOCYTE SUBSETS IN COLON WFFH ULCERATIVE COLITIS

ObjectiveTo gain new insight into the immune pathogenesis of ulcerative colitis (UC).MethodsImmunohistochemical techniques for T lymphocytes, B lymphocytes and macrophages were used to detect expression of different lymphocyte subsets in biopsy specimens of the colonic mucosa from 50 patients with ulcerative colitis and 5 cases of health control.ResultsThe numbers of T lymphocytes (CD45RO) and T h cell(OPD 4) were significantly higher in active UC (n=32) than inactive UC(n=18) or health control (P< 0.01 ).In comparison to controls, there was no difference in lamina propria B lymphocytes (CD20). Significant difference in macrophages was found between the patients with active UC and patients with inactive UC (P< 0.01 ).ConclusionT lymphocytes and macrophages are closely related to activity of UC and provide an immune mechanism for the participation in the acute effect events of tissue destruction. They were important components of immune response network of UC.
