以颈根部气管食管旁肿块为首发表现的食管癌

目的探讨以颈根部气管食管旁肿块为首发表现的食管癌的诊断和治疗.方法收集我科1992～1999年收治的上述食管癌患者11例,对其临床病史、影像学及病理资料进行回顾性分析.结果上述病例中,皆伴有声嘶,8例吞咽困难不明显;CT检查有其特征性改变;内窥镜检查为首选查找原发灶的方法;对只有单个颈根部气管食管旁淋巴结转移,原发灶情况尚可的胸段食管癌,建议选择包括手术治疗在内的综合治疗.结论胸段食管癌单发性颈根部气管旁淋巴结转移,可认为是区域淋巴结转移,若能及时诊断,这类病例有可能得到根治性治疗.

Esophageal Carcinoma with Inferior Cervical Tracheoesophageal Groove Mass as First Presentation Sign

Purpous: To discuss the diagnosis and treatment of esophageal carcinoma with inferior cervical tracheoesophageal groove mass as the first presenting sign. Methods: The clinical history, imagings and pathological datas of 11 cases treated in our hospital from 1992 to 1999 were retrospectively analyzed. Results: All the patients had hoarseness, dysphagia was unobvious in eight cases and none in three cases. (2) There were characteristic changes in CT examination. (3 ) Endoscopy was the first choice to pick out the primary lesion. (4) The treatment of the thoracic esophageal carcinoma with a single inferior cerical tracheoesophageal groove lymph node metastasis should be the composite treatment including surgery. Conclusion: Thoracic esophageal carcinoma with a single inferior cervical tracheoesophageal lymph node metastasis may be considered as local lymph node metastasis. If diagnosed in time, it will be curable.
