多西他赛联合顺铂化疗与三维适形放疗同步治疗局部晚期非小细胞肺癌的临床研究

摘    要:目的:探讨多西他赛加顺铂化疗同步三维适形放射治疗(three dimensional—conformal radiotherapy 3DCRT)的近期疗效、生存期及生活质量。方法:198例局部晚期非小细胞肺癌患者非随机分2组,化疗与放疗序贯组,化疗与放疗同步组。2组病例均采用3DCRT计划,照射剂量70~74Gy,2~2.5Gy/次,3次/周。同步组:102例,多西他赛40mg/m^2 d1、顺铂30mg/m^2 d1,在放射治疗开始时同步进行,每周1次,共用4~5次。序贯组:96例,多西他赛60mg/m^2 d1、顺铂80mg/m^2,d1,每3周1次,2周期后行放疗。结果:同步组和序贯组的总有效率分别为62.74%(64/102)和40.62%(39/96),差异有显著性(P〈0.01);中位生存时间分别为21.6个月,18.4个月,同步组优于序贯组,差异无显著性;两组毒性差异相似。结论:三维适形放疗联合周剂量多西他赛同步治疗局部晚期非小细胞肺癌近期疗效较好,能明显提高患者的生活质量,不良反应能为患者耐受。
