留桥切挂缝合术治疗复杂性肛瘘

目的探讨更适合复杂性肛瘘的手术治疗方法,提高治愈率,保护好肛门的功能,减少肛门并发症和后遗症。方法采用支管留桥、主管切挂缝合术96例与传统的切挂缝合术96例疗效对比。结果随访6个月～2年,两组病例在一次性手术治愈和复发第2次手术治愈方面差异无统计学意义(P>0.05),但两组平均愈合时间、疼痛持续时间、肛周潮湿瘙痒、肛门瘢痕面积、肛门部分失禁,肛管移位变形等方面差异有统计学意义(P<0.05)。结论留桥切挂缝合术治疗复杂性肛瘘在部分临床疗效上明显优于对照组,该手术方法具有治愈率高,痛苦小,愈合快,无明显畸形等优点。

Treatment of complex anal fistula with incision, thread-drawing, remain skinny bridge and suture

Objective To investigating the better surgical method to cure the complex anal fistula,then we can improve the curative effect,protect the anus function and reduce the anus complications and sequelae.Methods Compare the treatment of complex anal fistula with remain skinny bridge method in by-path,and incision,thread-drawing and selective suture in main branch in 96 cases and also 96 cases with traditional incision,thread-drawing and suture.Results After 6 months to 2 years’ follow-up,we found that there were no significantly differences in once cure rate and second operation cure rate after recurrent in two groups(P>0.05).But there were significantly differences in average healing time,pain duration,perianal moist and itching,scar area,partly incontinent,and canal deformation(P<0.05).Conclusion The treatment of complex anal fistula with incision,thread-drawing,remain skinny bridge and suture has advantages in high cure rate,little pain,fast healing and no abnormalities,etc.
