电视胸腔镜治疗进行性血胸临床分析

目的分析胸腔镜对外伤后血胸的治疗优势。方法 2007年5月～2010年9月我院对25例外伤后进行性血胸患者在胸腔镜下探查止血(胸腔镜组),2004年5月～2007年5月对21例同类患者行开胸探查止血(开胸组),比较两组手术时间、术后引流量、术后再出血率、镇痛药使用时间、住院时间、胸部并发症发生率。结果胸腔镜组中因胸膜腔粘连重影响操作和心包积血肺严重裂伤辅以小切口1例,中转开胸1例;胸腔镜组手术时间、术后引流量、置管时间、镇痛药使用时间、住院时间均较开胸组短(P<0.05)。结论胸腔镜治疗进行性血胸疗效确切、创伤小、恢复快、并发症少值得推广。

Clinical analysis of video-assisted thoracoscopic treatment for hemothorax

Objective Analysis of the advantage of thoracoscopy for the treatment of post-traumatic for hemothorax.Methods From 2007.5 to 2010.9,24 patients with post-traumatic for hemothorax were operated on with thoracoscopic for exploration and hemostasis,From 2004.5 to 2007.5,21 patients with post-traumatic for hemothorax were operated on with thoracotomy,two groups were compared operative time,postoperative drainage,postoperative bleeding rate,analgesic use,hospitalization time,incidence of chest complications.Results In VATS group,because of pleural adhesions re-operation and hemopericardium severe laceration of lung combined with a small incision in 1 case,1 case of transit thoracotomy;VATS group operation time,postoperative drainage,the amount of tube time,analgesic use,hospitalization time shorter than thoracotomy group(P<0.05).Conclusion Thoracoscopic treatment for hemothorax is effective,minimally invasive,quick recovery,fewer complications,is worth promoting.
