TVT-O术两种不同调整吊带张力方法疗效比较

目的 对比行经闭孔无张力阴道吊带术（TVT-O）时，用手术剪和用食指尖两种方法调整吊带张力治疗女性压力性尿失禁（stress urinary incontinence，SUI）的效果差异．方法回顾分析TVT-O术76例，术后随访3～12月．用手术剪调整吊带张力21例，用食指尖调整吊带张力55例．结果手术剪调节张力组和食指调节张力组患者术后尿失禁症状完全消失分别为19例（90．5％）、51例（92．7％）（P〉0．05），剩余病例症状均获显著改善．手术剪调节张力组发生排尿困难2例（9．5％），食指调节张力组1例（1．82％）（P〈0．01）．结论TVT-O术为治疗女性SUI安全有效的方法，使用食指调整吊带张力较传统方法可以有效地减少术后排尿困难发生率且并不降低治疗效果．

Comparison of Effects between Two Ways for Adjustment of ape Tension in TVT-O

Objective To compare the effects between using operating scissor and forefinger to adjust the tension of tape in TVT-O for treating female stress urinary incontinence （SUI）. Methods 76 cases underwent TVT-O procedure and followed up for 3 - 12 months were retrospectively reviewed. 21 patients were adjusted tension of tape by using operating scissor and other 55 patients were adjusted by using forefinger. Results 19 cases （90.5%） obtained total resolution of urinary incontinence in scissor-adjustment group and 51 cases （92.7%） were cured in forefinger-adjustment group （P 〉 0.05） , the others were partially cured. In scissor-adjustment group and in forefinger-adjustment group, urinary retention occurred in 2 cases （9.5%） and 1 cases （ 1.82% ） （P 〈 0.01 ）. Gorlclusion TVT-O is a safe and effective treatment for female urinary stress incontinence. Compared with the traditional method adjusting tape tension by using operating scissor, using forefinger can decrease the incidence of urinary retention without decreasing curative effect.
