保留幽门的胰十二指肠切除术疗效和并发症探讨

目的 探讨保留幽门的胰十二指肠切除术（PPPD）的手术疗效和手术并发症的发生率.方法 回顾性总结2012年5月至2013年5月30例因胰腺及壶腹周围恶性肿瘤行PPPD病人的手术体会和术后并发症发生情况,其中男性19例,女性11例,年龄51～75岁,平均61岁.本组PPPD平均手术时间200 min,平均出血量120 ml.胆总管下段癌18例,十二指肠乳头癌5例,壶腹癌4例,胰头癌3例,均经术后病理证实.术后观察指标：①围手术期死亡;②胆瘘,持续引流出富含胆汁的引流液大于10 ml,超过5d者;③胰瘘,指术后3～10 d手术放置的引流管,每天腹腔引流液大于50 ml,且含大量淀粉酶（连续3d测引流液淀粉酶＞血清淀粉酶3倍）者;④胃排空障碍,参考Bar-Natan标准;⑤十二指肠空肠吻合口漏,术后腹腔引流出胃肠内容物者;⑥腹腔或消化道出血,通过腹腔引流管或消化道失血,血红蛋白浓度低于80 g/L或24 h降低20 g/L,需最少2U血补充失血者;⑦腹腔感染,有腹部感染的临床症状且引流出脓性分泌物者.结果 总并发症发生率为26.7％ （8/30）,其中胃排空障碍2例,胰瘘1例,肺部感染及胸腔积液3例,切口感染2例.无围手术期死亡,无胆瘘、十二指肠空肠吻合口漏、消化道出血等并发症发生.结论 保留幽门的胰十二指肠切除术手术相对于传统的胰十二指肠切除术,可减少手术时间,减少术中出血量,是一种安全有效的手术方式.

Management of 30 cases of pylorus preserving pancreatoduodenectomy

Objective To explore the operative efficacies and complications of pylorus preserving pancreatoduodenectomy （PPPD）.Methods Retrospective reviews were conducted for surgical techniques and postoperative complications of 30 cases of pylorus preserving pancreatoduodenectomy.Results In the group of PPPD,the average operative duration was 200 minutes and average bleeding volume 120 ml.The total complication rate was 26.7％ （8/30）,including gastric emptying dysfunction （n =2）,pancreatic fistula （n =1）,infection of lung and pleural effusion （n =3） and wound infection （n =2）.There was no occurrence of perioperative mortality,biliary fistula,duodenal jejunal anastomotic leakage or hemorrhage of digestive tract.Conclusions Pylorus preserving pancreatoduodenectomy is both safe and efficacious.
