心血管病病人围术期心跳骤停原因分析与心肺复苏

目的探讨心血管病病人围术期心跳骤停的原因和总结心肺复苏成功的经验.方法回顾性分析20例心血管病病人心跳骤停的原因,并总结心肺复苏成功经验.结果15例病人复苏成功,5例死亡.心跳骤停的原因为急性心包填塞、大血管破裂、急性心肌梗死、严重酸血症和低钾血症、麻醉深度不够疼痛刺激、冠状动脉旁路血管移植术后移植血管闭塞.复苏成功的经验是对无气管插管的心跳骤停病人立即施行气管插管,坚持胸外按压和机械通气,大剂量肾上腺素的应用.结论心血管病病人围术期心跳骤停的原因较多,如果能及时发现,针对原因处理,复苏容易成功.

Retrospective analysis of cardiac arrest and cardiopulmonary resuscitation in 20 patients with cardiovascular diseases during perioperative period

Objective To review the cause of cardiac arrest patients during the perioperative period and summarize the experience of cardiopulmonary resuscitation. Methods A retrospective analysis of twenty cardiac arrest patients was carried out, experience of cardiopulmonary resuscitation were summarized . Results Fifteen arrest victims were successfully resuscitated, five died.The cause of cardiac arrest includes acute cardiac tamponade,rupture of great blood vessel, acute myocardial infarction, severe acidosis and hypokalemia, shallow anesthesia and pain stimulation ,acute bypass graft occlusion. Experience of cardiopulmonary resuscitation include instant intubating the arrest victims and ventilating with 100% oxygen, initiating closed-chest compression and high-dose epinephrine infusion. Aterial and central venous cannulation were established during resuscitation Conclusion The cause of cardiac arrest during perioperative period is complex, if the cause is found out as early as possible and dealt with accordingly, Successful resuscitation is easy to acquire.
