慢性胃炎患者不同中医证型水通道蛋白4表达的变化

目的:观察慢性胃炎不同证候与水通道蛋白4(AQP4)表达的相关性,探讨证候形成机制。方法:对158例慢性胃炎患者辨证分型,采用免疫组化法测定胃黏膜AQP4的表达。结果:AQP4表达总光密度(即阳性表达总量)从低到高的证型依次为:脾胃湿热→肝胃不和→脾胃虚弱→脾虚气滞→胃络瘀阻→胃阴不足;平均光密度(即表达部位的阳性强度)最低的是胃络瘀阻型,最高的是脾胃湿热型。结论:慢性胃炎从实证→因虚致瘀、虚实夹杂→虚证,AQP4表达逐渐增强,提示AQP4可能与中医虚实证候的形成有关,可将其作为判断病情演变的一项有用指标。

Study on expression of Aquaporin-4 in patients with chronic gastritis with different syndromes

Objective: To investigate the correlation between the expression of AQP4 in stomach mucosa of patients with chronic gastritis and different syndromes,and to probe the mechanism of different syndromes.Methods: 158 Cases of chronic gastritis were syndrome differentiated and the expression of AQP4 was dyed by immunohistochemistry.Results:The total optical density of AQP4 expression(total positive expression)in different syndromes from low to high was as follows: damp-heat of stomach and spleen→disharmony of liver and stomach→deficiency of spleen and stomach→spleen deficiency and qi stagnation→blood stasis in stomach→stomach yin deficiency;The lowest average optical density(expression parts of the positive intensity) was syndrome of blood stasis in stomach and the highest was syndrome of damp-heat of stomach and spleen.Conclusion: When the syndromes from excess to blood stasis caused by deficiency or mixed of excess and deficiency then to deficiency,the expression of AQP4 gradually increased which suggested that AQP4 may be related to the formation of TCM syndromes and it could be judged as a useful indicator of disease development.
