经颞下-小脑幕入路显微手术切除岩斜区肿瘤

目的探讨经颞下-小脑幕入路显微手术切除岩斜区肿瘤的疗效。方法回顾性总结采用经颞下-小脑幕入路显微手术切除的12例岩斜区肿瘤病例的临床资料。结果12例包括脑膜瘤7例、神经鞘瘤3例、胆脂瘤2例。肿瘤全切除8例，次全切除2例，大部切除2例。术后发生近期记忆障碍1例，肢体偏瘫1例，感觉性失语1例，治疗随访3个月后均痊愈。结论经颞下-小脑幕入路是切除岩斜区肿瘤的极佳入路，损伤小，并发症少，值得推广应用。

Microsurgical removal of petroclival tumors via subtemporal transtentorial approach

Objective To improve the therapeutic effect of the petroclival tumors removed via subtemporal transtentorial approach by microsurgical techniques. Methods Twelve patients with the petroclival tumors which were removed microsurgically via subtemporal transtentorial approach were retrospectively analyzed. The 12 cases comprised 7 meningiomas, 3 neuromas and 2 dermoid. Results Total removal, subtotal removal and partial removal of the tumors were achieved in 8 cases, 2 cases and 2 cases, respectively. Postoperative complications included short term memory disorder, paralysis of the contralateral limb and sensory aphasia, each of which occured in 1 case and got relieved 3 months later. Conclusions The subtemporal transtentorial approach is one of the optimal approaches to remove the petroclival tumors because of its mini-invasion and less complications, indicating for a wide application in clinic.
