急性脑梗死患者血清HbA1c水平及其与病情、神经功能和预后的相关性研究

摘    要:目的 探讨急性脑梗死患者血清糖化血红蛋白(HbA1c)水平与其病情、神经功能和预后的相关性.方法 对辽宁省人民医院2014年2月—2016年12月收治的急性脑梗死90例的临床资料进行回顾性分析,根据入院时血清HbA1c水平分为HbA1c正常组50例和HbA1c升高组40例.2组均给予常规治疗.观察2组神经功能缺损程度、改良Rankin量表评分,分析HbA1c水平与病情的关系.结果 HbA1c升高组合并糖尿病比例高于HbA1c正常组(P<0.01).脑梗死体积越大HbA1c水平越高(P<0.05).HbA1c升高组美国国立卫生院卒中量表和改良Rankin量表评分高于HbA1c正常组,神经功能缺损程度重于HbA1c正常组(P<0.01).结论 急性脑梗死患者HbA1c水平越高神经功能缺损程度越重,预后较差.HbA1c水平对于急性脑梗死病情的评判及预后的评估有重要的意义.

Abstract:Objective To investigate correlations between serum glycosylated hemoglobin (HbA1c) level with pathogenetic condition, neurological function and prognosis of patients with acute cerebral infarction. Methods Clinical data of 90 patients with acute cerebral infarction admitted during February 2014 and December 2016 was retrospectively analyzed, and the patients were divided into HbA1c normal group (n=50) and HbA1c elevation group (n=40). All patients underwent conventional therapy. In two groups, degrees of neurological function damage and scores of improved Rankin rating scale were observed, and correlation between HbA1c level and pathogenetic condition was analyzed. Re-sults Ratio of combined diabetes in HbA1c elevation group was significantly higher than that in HbA1c normal group (P<0. 01). The larger the cerebral infarction volume was, the higher the HbA1c level was (P<0. 05). In HbA1c ele-vation group, scores of NIHSS and improved Rankin rating scale were significantly higher, while degree of neurological function damage was worse than those in HbA1c normal group (P<0. 01). Conclusion In patients with acute cerebral infarction, the higher the HbA1c levels are, the worse degrees the neurological function damage and prognosis are, and therefore HbA1c level has a certain role in evaluation and prognosis of patients with acute cerebral infarction.
