摘    要:目的 探讨多b值磁共振弥散加权成像(MR-DWI)对宫颈癌的诊断价值.方法 选择陕西省安康市中心医院2014年1月—2016年10月确诊的宫颈癌100例,均接受1.5 T MR常规序列及多b值DWI扫描,通过双指数衰减模型计算慢速表观扩散系数(ADCslow)、快速表观扩散系数(ADCfast)及快速弥散所占容积分数(Ffast),观察其与宫颈癌临床病理特征的关系.结果 鳞癌ADCslow低于腺癌,ADCfast和Ffast值高于腺癌(P<0.05);不同分化程度鳞癌ADCslow比较差异有统计学意义(P<0.05),而ADCfast、Ffast值比较差异无统计学意义(P>0.05);不同FIGO分期、肿瘤直径、有无淋巴结转移患者ADCslow、ADCfast、Ffast值比较差异无统计学意义(P>0.05).结论 1.5 T MR多b值DWI相关参数对宫颈癌病理类型具有较高的诊断价值.

Abstract:Objective To investigate application value of multi-b value magnetic resonance diffusion weighted imaging ( MR-DWI) in diagnosis of cervical cancer. Methods A total of 100 patients with cervical cancer, who under-went 1. 5T MR routine sequence and multi-b value DWI scanning between January 2014 and October 2016, were recrui-ted in this study. Values of slow apparent diffusion coefficient ( ADCslow ) , fast apparent diffusion coefficient ( ADCfast ) and fraction of fast dispersion ( Ffast ) were calculated by double exponential decay model, and correlations between the values and clinically pathological characteristics of cervical cancer were observed. Results In patients with squamous cell carcinoma, ADCslow value was significantly lower, ADCfast and Ffast values were significantly higher than those in pa-tients with adenocarcinoma (P<0. 05). In squamous cell carcinomas patients with different differentiation degrees, there were significant differences in ADCslow value (P<0. 05), but there were no significant differences in ADCfast and Ffast val-ues (P>0. 05). There were no significant differences in ADCslow, ADCfast and Ffast values in patients with different FIGO stages, different tumor diameters and whether or not having lymph node metastasis (P>0. 05). Conclusion The 1. 5T MR multi-b value DWI parameters have a certain value in diagnosis of pathological type of cervical cancer.
