脓毒症患者血清降钙素原、C-反应蛋白水平及诊断意义

目的 探讨急诊脓毒症患者血清降钙素原(PCT)、C-反应蛋白(CRP)水平及诊断意义.方法 选取2014年12月-2015年9月收治的急诊脓毒症患者60例,根据病情将患者分为轻度脓毒症组40例和重度脓毒症组20例,另选取同期体检健康者30例作为正常健康组.通过检测3组血清PCT、CRP和血乳酸水平,分析PCT、CRP对急诊脓毒症的临床意义.结果 重度脓毒症组PCT、CRP及血乳酸水平均高于轻度脓毒症组和正常健康组(P＜0.05),且轻度脓毒症组高于正常健康组(P＜0.05).血清CRP和PCT水平与血乳酸水平呈现正相关性(P ＜0.001).CRP与PCT水平也呈现直线正相关性(P ＜0.001).结论 血清PCT、CRP水平能作为早期诊断脓毒症的指标,可准确评估患者病情严重程度,指导临床制定治疗方案.

Serum Procalcitonin and C-Reactive Protein Levels in Patients with Sepsis and Their Diagnostic Significances

Objective To investigate serum procalcitonin (PCT) and C-reactive protein (CRP) in patients with acute sepsis and their diagnostic significances.Methods A total of 60 patients with acute sepsis admitted during December 2014 and September 2015 were divided into mild sepsis group (n =40) and severe sepsis group (n =20) according to patient's condition,and another 30 healthy people taking medical examinations at the same period were selected as normal healthy group.Serum PCT and CRP levels were detected in three groups to analyze their significances of PCT and CRP for clinical significances of patients with acute sepsis.Results Serum PCT,CRP and lactic acid levels in severe sepsis group were significantly higher than those in mild sepsis and normal healthy groups (P ＜ 0.05),and the levels in mild sepsis group were significantly higher than those in normal healthy group (P ＜ 0.05).Serum CRP and PCT levels were positively correlated with serum lactate levels (P ＜ 0.001).CRP and PCT levels also showed a linear positive correlation (P ＜ 0.001).Conclusion Serum PCT and CRP levels can be used as indexes in early diagnosis of sepsis,accurately assess severity,guide clinicians and establish rational treatment plan for patients with sepsis.
