乳腺浸润性导管癌组织中ER、PR、Ki-67、p53表达与超声造影特征的相关性研究

摘    要:目的 探讨乳腺浸润性导管癌组织中雌激素受体(ER)、孕激素受体(PR)、Ki-67、p53表达与超声造影特征的相关性.方法 选取2014年1月—2016年10月符合研究要求的乳腺浸润性导管癌患者78例,收集患者临床资料.所有患者均行超声造影检查、病理检查及相关免疫组织化学检测.分析患者癌组织中ER、PR、Ki-67、p53表达与超声造影特征的相关性.结果 肿块边缘呈毛刺征和有淋巴结转移患者的ER、PR、Ki-67、p53阳性表达率分别明显高于无毛刺征和无淋巴结转移患者(P<0.05).在等-低增强患者的ER和PR阳性表达率显著低于高增强患者(P<0.05).增强均匀患者p53阳性表达率显著低于增强不均匀患者(P<0.05);有灌注缺损患者Ki-67阳性表达率高于无灌注缺损患者(P<0.05).在不同彩色多普勒超声血流显像(CDFI)血流分级患者ER、PR、Ki-67、p53阳性表达率比较存在明显差异,随CDFI血流分级的增高其阳性表达率随之上升(P<0.05).结论 乳腺浸润性导管癌组织中ER、PR、Ki-67、p53表达与超声造影特征存在一定相关性,超声检查可为临床评估患者病情及治疗提供更多依据.

Abstract:Objective To investigate correlations between expressions of estrogen receptor (ER), progesterone receptor ( PR) , Ki-67 and p53 in breast invasive ductal carcinoma tissues and characteristics of contrast-enhanced ultra-sound. Methods Clinical data of 78 patients with breast invasive ductal carcinoma, who met study requirements admit-ted during January 2014 and October 2016, were collected. All patients underwent contrast-enhanced ultrasonography, pathology and related immunohistochemistry examinations. Correlations between ER, PR, Ki-67, p53 expressions and characteristics of contrast-enhanced ultrasonography were analyzed. Results Positive expression rates of ER, PR, Ki-67 and p53 were significantly higher in patients with burr in mass edge and lymph node metastasis than those in patients with-out burr in mass edge or lymph node metastasis (P<0. 05). Positive expression rates of ER and PR in patients with e-qual-low enhancement image were significantly lower than those in patients with high enhancement image ( P<0. 05 ) . Positive expression rate of p53 expression in patients with enhanced and uniformity image was significantly lower than that in patients with enhanced and nonuniform image (P<0. 05). Positive expression rate of Ki-67 in patients with perfusion defects was significantly higher than that in patients without perfusion defects (P<0. 05). There were significant differ-ences in positive expression rates of ER, PR, Ki-67 and p53 in patients with different color Doppler flow imaging ( CDFI) blood flow classifications, and the positive expression rates were increased with increasing CDFI blood flow classification (P<0. 05). Conclusion ER, PR, Ki-67 and p53 expressions in breast invasive ductal carcinoma tissues have a cer-tain correlation with characteristics of contrast-enhanced ultrasonography. Ultrasonography can provide more evidence for clinical evaluation of patients' condition and treatment.
