摘    要:目的 评价低蛋白饮食对糖尿病肾病的治疗效果.方法 选取2015年3月-2017年1月解放军白求恩国际和平医院收治的糖尿病肾病64例,随机分为研究组及对照组,每组32例.2组均进行常规饮食指导,研究组在此基础上给予优质低蛋白饮食,治疗周期为3个月.观察并比较2组治疗前后体重指数(BMI)、平均动脉压(MAP)、血红蛋白(Hb)、白蛋白(ALB)、尿素(BUN)、肌酐(Cr)、尿酸、肾小球滤过率(GFR)、24 h尿白蛋白、糖化血红蛋白(HbAlc)、空腹血糖(FPG)、餐后2h血糖(2 h PG)、血清甘油三酯(TG)、血清总胆固醇(TC)水平.结果 研究组BMI、MAP、Hb、ALB、BUN、Cr、尿酸、GFR、24 h尿白蛋白、HbAlc、FPG、2hPG、TG、TC均优于治疗前及对照组(P<0.05).结论 优质低蛋白饮食可改善GFR,减轻肾小球硬化,提高血清ALB水平,延缓肾功能损伤.

Abstract:Objective To evaluate effect of low protein diet in treatment of patients with diabetic nephropathy.Methods A total of 64 patients with diabetic nephropathy during March 2015 and January 2017 were randomly divided into research group and control group (n =32 for each group).All patients received regular diet guidance,and then research group was given a low-protein diet for 3 months.Body mass index (BMI),mean arterial pressure (MAP),hemoglobin (Hb),albumin (ALB),blood urea nitrogen (BUN),creatinine (Cr),uric acid,glomerular filtration rate (GFR),24 h urinary albumin and glycated hemoglobin (HbA1c),fasting plasma glucose (FPG),2 h postprandial blood glucose (2 h PG),serum triglyceride (TG) and serum total cholesterol (TC) levels were observed and compared before and after treatment in two groups.Results BMI,MAP,Hb,ALB,BUN,Cr,uric acid,GFR,24 h urinary albumin,HbA1 c,FPG,2 h PG,TG and TC in research group were superior to those before treatment and in control group (P < 0.05).Conclusion High quality low-protein diet can improve GFR,alleviate glomerular sclerosis,improve serum ALB level and delay renal function injury of patients with diabetic nephropathy.
