膀胱移行细胞癌中凋亡抑制因子survivin的表达及意义

目的研究凋亡抑制因子survivin在BTCC中的表达及其与肿瘤分级、分期、复发之间的关系.方法采用免疫组化法(ABC法),对52例BTCC和10例正常膀胱中survivin的表达情况进行检测,分析其在膀胱癌和非癌膀胱组织中的表达以及与肿瘤病理学分级、分期和患者复发情况的关系.结果survivin的表达随BTCC的病理分级、分期的增高而相应增高,Ⅰ、Ⅱ、Ⅲ级中表达率分别为60.0%、70.8%、100%,浅表性(Tis～T1)、浸润性(T2～T4)中表达率分别为43.5%、93.1%,初发BTCC、复发BTCC表达率为59.4%、90.0%(P＜0.05).结论细胞凋亡抑制因子surwivin在BTCC组织中表达上调,提示survivin可能通过抑制细胞凋亡,对BTCC的发生发展起重要作用;survivin随BTCC病理分级、分期增高而表达增高,并与肿瘤复发有关,survivin有可能作为判断分级、分期,监测复发的指标;survivin可能为膀胱癌基因治疗提供新的合理靶点.

Expression and significance of survivin in bladder transitional cell carcinomas

Objective: To study the expression of survivin in BTCC and its relationship between tissue grade,pathology stage, recurrence of bladder transitional cell carcinomas (BTCCs). Methods: 52 bladder transitional cell carcinomas and 10 normal bladder mucosas were used to detect the expressions of survivin using immunohistochemistry. Results:The expression of survivin in 52 BTCCs is higher than that in 10 normal bladder tissues (P ＜ 0.05).The positive rate is 71.1%. The expression level of survivin is elevated as the grade and stage increased in BTCC.The positive rates in grade Ⅰ、Ⅱ、Ⅲ are 60.0% 、70.8% 、100%, in stage of Tis - T1 、T2 ～ T4 are 43.5% 、93.1%;and in primary、 recurrent cases are 59.4% 、90.0%, respectively. All of the above data are statistically significant at a level of P ＜ 0.05. Conclusions: The up - regulated expression of survivin in BTCC suggests that survivin plays an important role in tumor occurrence and progress through inhibiting cell apoptosis. Besides, the higher the grade and stage , the higher the expression level of survivin. Furthemore, Survivin is correlated with recurrence of BTCC.Therefore, we concluded that Survivin might be the indexes for identifying grade, stage and recurrence of BTCC.Receivcd date:2003-9-15Survivin can provide new appropriate target for gene therapy to BTCC.
