基本药物制度对基层医疗机构运行和费用负担的影响分析

基于全国431家基层医疗机构的调查,对基本药物制度实施后基层医疗机构收支结构、门诊量及居民医疗费用负担的变化情况进行评估,发现国家基本药物制度的实施加强了政府对基层医疗机构的财政补助,但收入和支出结构仍存在不协调,制度实施后药价的下降及对药品配置和使用的限制引起基层机构服务量普遍上升,实施基本药物制度的基层医疗机构次均费用得到一定程度的降低。

Impact of the essential medicines policies on operation and expenditure burden at primary health care institutions

Based on the survey of sampled 431 primary health care institutions, this study analyzed the changes of financial structure, service volume and burden of expenditure at primary health care institutions after implementing the national essential medicines policies. The study found that the government funding played a more important role in implementing essential medicines policies. Decreasing drug prices and the limitation on drugs utilization have led to the increase of service amount and, to some extents, the decrease of medical expenditure per visit at primary health care institutions.
