灯盏细辛注射液对急性脑梗死患者神经功能状态及脑血流的影响

摘    要:目的:探讨灯盏细辛注射液对急性脑梗死患者神经功能状态及脑血流的影响。方法:将2012年6月~2013年12月本院收诊的急性脑梗死患者随机分成对照组和观察组,各85例,对照组采用基础治疗,观察组在基础治疗的前提下加载灯盏细辛注射液。治疗前及治疗后第7、28天,采用NIHSS评分和Fugl—Meyer评分对两组患者的神经功能状态进行评估,同时检测血清中神经功能相关细胞因子水平;运用彩色多普勒超声仪检测治疗前后两组大脑中动脉血流状态差异;比较治疗前后两组血液学指标差异。结果:治疗后第7、28天,观察组NIHSS评分显著低于对照组,Fugl—meyer评分则显著高于对照组,血清神经功能相关细胞因子水平超敏C反应蛋白(hs—CRP)、磷酸酯(PA)、β-淀粉样蛋白(AB)和糖基化终产物(AGEs)水平均显著低于对照组(P〈0.05);观察组患者平均收缩期峰值流速(PSV)、舒张末期血流速度(EDV)、血流速度(Vm)、血管搏动指数(PI)显著高于对照组,而阻力指数(RI)则显著低于对照组(P〈0.05)。治疗7、14d后观察组血浆黏度(PV)、血小板计数(PLT)显著低于对照组,凝血时间(TT)、凝血酶原时间(PT)显著长于对照组(P〈0.05)。结论:灯盏细辛注射液能有效降低急性脑梗死患者血液黏稠度,抑制血小板聚集,从而改善大脑血流供应以发挥治疗神经功能缺损的功效。

Abstract:Objective: To investigate the effect of erigeron injection on the neurological status of patients with acute cerebral infarction and cerebral blood flow. Methods. Patients with acute cerebral in- farction admitted from June 2012 to December 2013 were randomly divided into two groups, the control group and observation group (n=85 cases in each group). The control group received basic treatment, and the observation group was treated with load fleabane injection on the basis of basic treatment. Before treat-ment and 7, 28 days after treatment, the NIHSS score and Fugl-Meyer score for the two groups of pa- tients with neurological status was assessed, and serum cytokines in neurological level was detected. The color Doppler ultrasound was used to detect the differences in arterial blood flow of the brain between the two groups. The hematology differences for the two groups before and after treatment were compared. Results. 7 and 28 days after treatment, the NIHSS scores in the observation group were significantly lower than the control group, and Fugl-meyer score was significantly higher than the control group. The serum levels of cytokines related neurological hs-CRP, PA, Aβ and AGEs were significantly lower than the con- trol group, and the PSV, EDV, Vm, PI in the observation group were significantly higher than the con- trol group, while the RI was significantly lower than the control group (P〈0.05). 7, 14 days after treat- ment, the PV, PLT in the observation group was significantly lower than the control group, TT, PT was significantly longer than the control group (P〈0.05). Conclusions. Fleabane injection can effectively re- duce blood viscosity in patients with acute cerebral infarction, inhibit platelet aggregation, thereby im- prove blood supply of brain.
