摘    要: 目的 探讨实时三维超声心动图(RT-3DE)定量评价慢性心力衰竭(CHF)患者左心室收缩同步性的临床价值。 方法 将研究对象分为心衰组(41例)和正常对照组(30名),并行TDI和RT-3DE检查,应用QLab软件分析TDI及三维全容积数据,对两种检查方法所获结果进行统计学分析。 结果 两组患者年龄和心率差异均无统计学意义(P>0.05),左心室射血分数及QRS波差异有统计学意义(P<0.05)。两种评价方法的收缩非同步性指数(SDI)差异有统计学意义(P<0.05)。两种评价方法的SDI呈中度正相关,具有较高的一致性。 结论 RT-3DE可定量分析左心室收缩同步性,为心脏再同步化治疗术前筛选心力衰竭患者提供一种简单、快捷的新方法。

Abstract:Objective To explore the clinical value of real-time three-dimensional echocardiography (RT-3DE) on qualitative evaluation of intra-left ventricular synchronism in patients with chronic heart failure (CHF). Methods Forty-one patients with CHF were included in CHF group, while 30 normal subjects were enrolled in control group (n=30). TDI and RT-3DE data were acquired with an iE33 system and analyzed by the QLab software. Results No difference of age or heart rate was found (P>0.05), while left ventricular ejection fraction and QRS waves were significantly different between the two groups (P<0.05). Systolic dyssynchrony index (SDI) of the two evaluation methods were significantly different (P<0.05). Moderate positive correlation and higher concordance were documented between the two methods in evaluating SDI. Conclusion RT-3DE can quantify global left ventricular synchronism, may be used as a simple and quick method to identify chronic heart failure patients before CRT.
