卡前列甲酯栓含服预防剖宫产术后出血的临床研究

目的 观察剖宫产术中舌下含服卡前列甲酯栓预防产后出血的效果。方法 选择200例剖宫产者，随机分为3组；A组为卡前列甲酯栓组；B组为卡前列甲酯栓加催产素组；C组为催产素组。A组70例，术中打开子宫浆膜层时取卡前列甲酯栓1mg舌下含服。B组70例，术中打开子宫浆膜层时取卡前列甲酯栓1mg舌下含服。胎儿娩出后宫体肌肉注射催产素20IU。C组60例，胎儿娩出后宫体肌肉注射催产素20IU。再静脉滴注催产素20IU。以上各组观察术中及术后2小时内出血量。结果 术中及术后2小时出血量，卡前列甲酯栓组与催产素组比较，差异有极显著性（P〈0．01）。卡前列甲酯栓组与卡前列甲酯栓加催产素组差异无显著性（P〉0．05）。结论 卡前列甲酯栓舌下含服促进子宫收缩作用强于催产素，能很好地预防剖宫产术后宫缩乏力性出血，且卡前列甲酯栓用药方便、安全、值得推广。

Clinical Study on Prevention of Postpartum Bleeding in Caesarean Section by Sublingual Administration of Methyl Carprost Suppositories

OBJECTIVE To observe the effects of Methyl Carprost Suppositories on reduction of postpartum bleeding in caesarean section. METHODS Two hundred caesarean sections were randomized into three groups: Carprost group(n=70): 1mg was given sublingual when open the uterine; Carprost+oxytocin group (n=70): 1mg was given sublingual when open the uterine and 20 IU oxytocin was injected into uterine muscle after delivery of baby. Oxytocin group(n=60): 20 IU oxytocin was injected into uterine muscle and 20 IU intravenous injection immediately after delivery of baby. The amount of bleeding within 2 hours after delivery was measured. RESULTS Measuring the amount of bleeding within 2 hours after delivery. The difference was significant (P<0.01)between Carprost group and oxytocin group; there was no differences between Carprost group and Carprost+oxytocin group(P>0.05). CONCLUSION Carprost is more effective in reducing of postpartum bleeding in caesarean section than that of oxytocin.
