活体肝移植术前三维超声测量供者肝脏体积与术中肝脏体积和质量的关系研究

目的 探讨活体肝移植术前三维超声测量供者右半肝体积与术中测得供肝体积和质量的相关关系.方法 术前使用三维超声分别测量供者不含肝中静脉和含肝中静脉的右半肝体积.术后根据采用的术式,使用相应的测量值作为供肝的术前体积(V_(术前)),并将供者分为2组.术中测得供肝的体积(V_(术中))和质量(W_(术中)).计算V_(术前)和V_(术中)的偏差,并对V_(术前)与V_(术中)和W_(术中)进行相关与回归分析.结果 不含肝中静脉组,V_(术前)与V_(术中)的体积偏差为-3.44%±14.97%,V_(术前)与V_(术中)的相关系数r=0.680,P<0.001,V_(术前)与W_(术中)的相关系数r=0.682,P<0.001;含肝中静脉组,V_(术前)与V_(术中)的体积偏差为6.92%±17.11%,V_(术前)与V_(术中)的相关系数r=0.475,P=0.001,V_(术前)与W_(术中)的相关系数r=0.565,P<0.001.结论 三维超声测量供者肝脏体积与术中肝脏体积和质量存在良好的线性相关性.

Research on Relationship between the Preoperative Volume of the Donor's Liver and its Intraoperative Volume and Weight in LDLT by Three-dimensional Ultrasonography

Objective To evaluate the correlation between preoperative volume measured by three-dimensional ultrasonography(3DUS) and its intraoperative volume and weight of graft in living donor liver transplantation(LDLT).Methods Volumes of right hepatic lobes without and with middle hepatic vein (MHV) of donors were measured by 3DUS respectively before operation. According to the operation manner, we took relevant measured value as the preoperative volume (V_(preop)) of graft after operation and divided donors into 2 groups.Intraoperatively the volume (V_(intraop)) and weight (W_(intraop)) of graft were achieved.We calculated the deviation of V_(preop) from V_(intraop) and analysed the correlation and regression among V_(preop),V_(intraop) and W_(intraop).Results The group without MHV: the volume deviation was -3.44%±14.97%.Correlation coefficient between V_(preop) and V_(intraop) was 0.680,P<0.001.Correlation coefficient between V_(preop) and W_(intraop) was 0.682,P<0.001.The group with MHV: the volume deviation was 6.92%±17.11%.Correlation coefficient between V_(preop) and V_(intraop) was 0.475,P=0.001.Correlation coefficient between V_(preop) and W_(intraop) was 0.565,P<0.001.Conclusions The study shows the 3DUS volumetric measurement has a significant correlation with the intraoperative volume and weight of graft.
