胸腔镜辅助下腋下小切口造路治疗食管癌

目的介绍胸腔镜辅助下腋下小切口进路治疗食管癌。方法侧卧位，左臂上举悬吊，双腔管单肺通气复合麻醉，横切口或纵切口6—8cm。肋间切口略大于皮切口，颈部切口3cm吻合。结果小切口进路手术时间较传统手术操作时间缩短，术后病人咳痰有力，吸氧时间短，下床活动早。接受术后放、化疗早，围手术期管理简单。未发现与胸切口相关的并发症。结论小切口进路治疗食管癌是一种可选择的治疗方法。

Treatment of cancer of the esophagus using mini-incision under armpit

Objective To introduce mini-incision to treat cancer of the esophagus. Methods Let patients lie down on his right side, rise his left upper arm and hang it. Intersection or longisection incision which is 6 - 8cm long with its intercostal space open bigger than its skin using combined anesthesia method with double-channel signal lung ventilation, and anastomosis on his neck. Results With this method, the operating period is shorter, so he can cough more powerfully, get up to do his exercises earlier and stop his oxygen-inhalation earlier after operation. He can also accept radiotherapy or chemotherapy earlier. And the management is simpler after operation. There is no complication related to the incision found. Conclusion The method can be chosen to treat cancer of esophagus.
