开腹胆道探查术中胆道镜经胆囊管取石经验总结

目的总结开腹胆道探查术中胆道镜经胆囊管完成胆道探查取石的技巧及临床效果。方法回顾分析我院2004年1月至2006年11月开腹经胆囊管胆道探查术86例患者的临床资料。结果根据术前磁共振胰胆管成像（MRCP）进行适应证筛选后,全组患者均成功完成经胆囊管胆道探查取石,无严重术后并发症。全组病例平均手术时间（72.2±29.4）min,术中经胆囊管取石时间（9.8±6.8）min,平均术后住院时间（6.8±10.8）d。结论经过合理筛选的病例可以通过胆囊管途径完成胆道探查取石,治疗效果较胆道切开留置T管有明显优势。

The experiences of transcystic common bile duct exploration in Open surgery

Objective To summarize the clinical efficacy and experience of open trans-cytic common bile duct exploration （OTCBDE） with choledochoscope. Methods The clinical materials of 86 cases with OTCBDE was analyzed retrospectively from Jan 2004 to Nov 2006 in our hospital. Results Based on the pre-operation MRCP, all cases met the indications for transcystic approach were successfully performed, without any severe complications. The mean operative time and post-operative hospital stay were 72.2±29.4 minutes and 6.8±10.8 days. The average time of transcystic stone extraction with choledochosocpe was 9.8±6.8 minutes. Conclusion With appropriate indications, transcystic approach is feasible for the management of common bile duct stones, and has superior clinical efficacy when compared with choledochotomy.
