摘    要:目的:探讨尿微量蛋白检测对过敏性紫癜肾损害的早期诊断价值。方法:采用放射免疫分析分别检测25例单纯性过敏性紫癜(AP)、27例紫癜性肾炎(PN)患儿和32例正常儿童(对照组)尿微量白蛋白(Alb)、免疫球蛋白G(IgG)、β2微球蛋白(β2-m)含量,并同期检测常规肾功能指标。结果:正常对照组与AP组、PN组的尿系列蛋白含量比较差异均有统计学意义(P〈0.01),AP组与PN组比较差异亦有统计学意义(P〈0.01),而三组BUN、Cr均在正常值范围。结论:尿微量蛋白在过敏性紫癜肾功能受损时较肾功能指标BUN、Cr改变出现早,可作为早期诊断肾功能损害的敏感指标。

Abstract:Objective To study the early diagnostic value of determination of urinary excretion amourt of proteins for renal damage in pediatric patients with AP. Methods Morning arine specimen contents of albumin, IgG and β2 - m ( with RIA) as well as serum BUN and creatinine levels were measured in 25 pediatric patients with simple AP, 27 pediatric patients with purpura nephritis (PN) and 32 controls. Results The urinary contents of proteins in all the patients were significantly higher than those in controls ( P 〈 0.01 ). Furthurmore, the urinary contents of proteins in patients with PN were significantly higher than those in patients with AP. However, the serum levels of BUN and creatinine in the three groups were all within normal range and not much different ( P 〉 0.05 ). Conclusion Changes of urinary excretion of proteins occurred much earlier than changes of BUN and creatinine in purpura patients complicated with renal involvement and might be used as an indicator for early diagnosis.
